Purpose: By comparing the changes of TCM syndrome score,CMSS score and VAS score before and after treatment,as well as the changes of serum PGF2? level,PGE2 level and PGF2?/PGE2 value,to evaluate the time-effect relationship of moxibustion with drug separation in the treatment of primary dysmenorrhea with cold coagulation and blood stasis syndrome.Methods: The subjects of this study were patients who visited the gynecological outpatient clinic and ward of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from July 2018 to November 2019.Seventy-two patients with primary dysmenorrhea of cold coagulation and blood stasis syndrome who met the inclusion criteria were divided into two groups,A and B,with 36 patients in each group.They were treated with moxibustion for 4 pillars each day,5 days before the onset of menstruation,3 or 7 consecutive days for each menstrual cycle,and were both treated for 3 consecutive menstrual cycles in total.The changes of TCM syndrome score,CMSS score and VAS score before and after treatment,as well as the changes of serum PGF2?,PGE2 level and the value of PGF2?/PGE2 were compared between the two groups,so as to further determine the clinical efficacy.Results:1.Comparison of therapeutic effect between groups after treatment:After treatment,the total effective rate of group A was 94.29%,and that of group B was 91.67%.There was no significant difference between the two groups(P>0.05);after treatment,CMSS score and VAS score in the two groups both decreased,but there was no significant difference between the two groups(P>0.05).It shows that the therapeutic effect of moxibustion with medicine separation on primary dysmenorrhea with cold coagulation and blood stasis syndrome is equivalent to that of 7 days under the same number of pillars.2.Comparison of prostaglandins level between groups after treatment:After treatment,the level of serum PGF2? and the value of PGF2?/PGE2 of the two groups decreased,and there was no significant difference between the two groups(P>0.05);after treatment,the serum PGE2 level of the two groups had no significant change,and there was no significant difference between the two groups(P>0.05).The results showed that the effect of moxibustion on the treatment of primary dysmenorrhea with cold coagulation and blood stasis syndrome is equivalent to that of 7 days under the same number of pillars.3.Comparison of therapeutic effect before and after treatment in the group:After treatment,the TCM syndrome score,CMSS score and VAS score of the two groups were lower than those before treatment,and the serum level of PGF2? and PGF2?/PGE2 value were lower than those before treatment,the difference was statistically significant(P<0.01);there was no significant change in serum PGE2 level of the two groups after treatment,and the difference was not statistically significant(P>0.05).The results showed that the two groups can effectively improve the symptoms of dysmenorrhea,as well as abdominal pain,can effectively reduce the level of serum PGF2? and the value of PGF2?/PGE2,but there is no significant effect on changing the level of serum PGE2.Conclusion: Medication-separated moxibustion has magnificent clinical effect on primary dysmenorrhea with cold coagulation and blood stasis syndrome,can significantly alleviate the degree of TCM syndrome,dysmenorrhea symptoms and abdominal pain,reduce TCM syndrome score,CMSS score and VAS score,and reduce serum PGF2? level and PGF2?/PGE2 value during menstruation.With the same number of pillar per day,there was no significant difference in the efficacy between 3 days of treatment and 7 days of treatment.Therefore,in contrast,4-pillar 3-day moxibustion treatment is more convenient and safer,with no toxic side effects,which should be further promoted in clinical treatment.
