Objective:Based on the inheritance of TCM experience,we use randomized controlled trial method to carry out a clinical reseach of "five facial acupoints" catgut implantation treating refractory facial paralysis compared with electroacupuncture.To evaluate the clinical efficacy,safety and advantages of "five facial acupoints" catgut implantation and discuss its theoretical basis,so as to provide scientific clinical practice basis for clinical spread.Methods: 66 included subjects were randomly divided into embedding group and electroacupuncture group.Embedding group was treated with "five facial acupoints" catgut implantation,once every two weeks in the affected side and only one time in the healthy side(one week after the first treatment),12 weeks for one course,7times in total.Electroacupuncture group was treated with electroacupuncture therapy,three times a week,12 weeks for one course,36 times in total.In this research,we use H-B,facial nerve function scoring standard,SAS and SDS to evaluate the clinical efficacy.In the meantime,record the adverse events,economic cost and time cost to evaluate the safety and advantages.Results:1.H-B:H-B rates in both groups were lower than those before treatment,the difference was statistically significant(P<0.05).There was no significant difference between two groups after treatment(P>0.05).2.Facial function score:(1)Intragroup Comparison:After treatment,embedding group's static scores and complication scores were lower,dynamic scores were higher than those before treatment,electroacupuncture group's static scores were lower and dynamic scores were higher than those before treatment,the difference was statistically significant(P<0.05).Electroacupuncture group's complication scores were similar to those before treatment(P>0.05).(2)Comparison between groups : After treatment,embedding group's static score were lower and the dynamic scores were higher than electroacupuncture group,the difference was statistically significant(P<0.05).Two groups' complication scores were similar(P>0.05).3.SAS,SDS:SAS,SDS scores in both groups were lower than those before treatment,scores in embedding group were lower than electroacupuncture group,thedifference was statistically significant(P<0.05).4.Clinical efficacy:The total effective rate was 80.00% in embedding group and64.29% in electroacupuncture group.The clinical efficacy of embedding group was better than electroacupuncture group(P<0.05).5.Safety evaluation:The safety evaluation of both groups was level II.6.Cost evaluation:The CER and time cost in embedding group were lower than electroacupuncture group.Conclusion: The research suggests that "Five facial acupoints" catgut implantation is an effective method for the treatment of refractory facial paralysis.The static facial symmetry,facial muscle movement and clinical efficacy in embedding group were absolutaly better than electroacupuncture group.With the improvement of facial nerve function,the patients' bad emotions such as anxiety and depression also reduced.In addition,"Five facial acupoints" catgut implantation also has the advantages of good acceptance,safety and economic benefits,which is worthy for clinical application.
