Objective: To observe the clinical efficacy and safety of Huayu Jiedu Decoction combined with urea ointment in treating apatinib-induced toxic-stasis type of hand-foot syndrome. Methods: 60 patients who met the inclusion criteria were randomly divided into treatment group(Huayu Jiedu Decoction +urea ointment)30 cases and control group(vitamin B6 + urea ointment)30 cases, according to the random number table method.Observing for 2 courses of treatment(28days),and recording the classification of hand foot syndrome,TCM syndrome points,KPS scores and adverse reactions of the two groups.Datas were analyzed by the software 25.0.Results:(1)Comparison of general data: The general conditions(gender, age, clinical stage)and routine examinations(blood routine, liver and kidney function)of the two groups of patients, which was not statistically significant.(2)Comparison of curative effects of hand foot syndrome between the two groups of patients before and after treatment.27 cases in the treatment group were evaluated as effective(22 cases were cured,5 cases were effective),3 cases were evaluated as ineffective, and the effective rate was 90%.20 cases in the control group were evaluated as effective(14 cases were cured,6cases were effective),10 cases were evaluated as ineffective, and the effective rate was 66.66%.The treatment group was better than thecontrol group, the difference was statistically significant(P<0.05).(3)Comparison of the clinical symptoms of TCM in the two groups before and after treatment.26 cases in the treatment group were evaluated as effective(18 cases were cured,6 cases were effective,2 cases were sub-effective),5 cases were evaluated as ineffective, and the effective rate was 86.66%,while 18 cases in the control group were evaluated as effective(9 cases were cured,7cases were effective,2 cases were sub-effective),12 cases were evaluated as ineffective, and the effective rate was 60%.The treatment group was significantly better than the control group, and the difference was statistically significant(P < 0.05).(4)There was no significant difference in the KPS scores between the two groups before and after treatment(P> 0.05).(5)Safety and adverse reactions:There was no significant difference in blood routine, liver and kidney function among the two groups after treatment(P> 0.05).During the treatment, the vital signs of the two groups were stable, there were no significant differences in safety indicators, and there were no adverse reactions such as nausea and vomiting, diarrhea, indicating that the medication was safe without toxic side effects. Conclusion: Huayu Jiedu decoction combined with urea ointment and vitamin B6 combined with urea ointment can effectively improve the classification and TCM syndrome scores of hand foot syndrome caused by apatinib. However, compared with vitamin B6 combined with urea ointment,Huayu Jiedu decoction combined with urea ointment is more effective in reducing the severity of HFS andimproving the clinical symptoms of traditional Chinese medicine. Also,it does not increase the blood, liver and kidney toxicity, which indicated that Huayu Jiedu decoction combined with urea ointment is safe and effective in treat ing hand-foot syndrome.
