Research On The Integration Of Basic Medical Insurance System For Urban And Rural Residents In Yunnan

In 2003,the New Rural Cooperative Medical policy was promulgated and started providing rural inhabitants with basic medical insurance in China.Afterwards,unemployed urban inhabitants has also been covered by another healthcare system called basic medical insurance for urban residents,which was established in 2007.From then on,a full coverage of healthcare system has been completed in China.With the deepening reform and the rapid development in Chinese economy and society,both urban and rural medical security system emerged various problems in facing the new conditions.The problems caused by "fragmentation" can't be ignored especially.Because of this,integrating the urban residents' basic medical insurance system and the NCMS and establishing a unified basic healthcare system,including rural and urban inhabitants,is a major initiative to promote the healthcare system reform.This unified healthcare service may assure urban and rural residents have equally access to basic medical insurance rights and promote social fairness and justice.I consider it as an important step to promote social welfare,and I believe it will benefit economy and social coordinate development in China.This research based on the Southwestern Borderland of China-Yunnan Province.I attempted to analyse the integrated healthcare system,which including both urban and rural inhabitants in Yunnan province,from a welfare sociology perspective.There are several research methods has been used in this research,including questionnaire survey,case interview,literature research and comparative study.First section,I summarised the develop process and the main policy contents of the Urban basic medical insurance and the NCMS in Yunnan province,analyzed the problems and the disadvantages in separated healthcare system and pointed out the importance of integrated healthcare system.Second section,I discussed how theories,including urban and rural theory,basic integration theory,the bottom line equity theory,the theory of universal access,enlighten the integrated healthcare system in Yunnan.Third section,I compared Kunming's healthcare system with Shandong,Chongqing and Chengdu's urban and rural basic medical insurance system in respective experience of integration.Fourth section,I analyzed people's willingness in integrated healthcare system,which based on the fieldwork data from Kunming and Honghe city.Fifth section,I considered the restricted factors,which involves economy,policy,population,thought,talent and technology in Yunnan province.These factors have influenced the integration of healthcare policy in urban and rural areas in Yunnan.The sixth section,some policy suggestions have been given for the integration of urban and rural healthcare system in Yunnan.These policy suggestions consist of 1)the general ideas and the basic principles of the integration of urban and rural residents' basic medical insurance system;2)the main factors need to be of integrated,including unified coverage,financing policies,security benefits,information system;3)suggesting the Human Resources and Social Security Department take over the urban and rural healthcare management,and then integrate the healthcare at a municipal level,or a provincial level(if conditions are allowed);4)listing the main measures can be taken by the Government in Yunnan,including to strengthen government's functions,establish mechanisms for multi-sectoral collaboration and promote the awareness of integration.
