In the report of the 19 th National Congress of the Communist Party of China,it was pointed out that the main contradictions in the current society have been transformed into unbalanced and inadequate development.This imbalance is not only reflected in the imbalance of overall development between urban and rural areas and regions,but also in the imbalance of economic and social construction.It is the core value orientation of social construction to coordinate regional development and promote social equity,which is also the internal requirement of socialism with Chinese characteristics.As the system guarantee of maintaining social fairness and justice,the degree of equalization of public service essentially reflects the transformation of social fairness from form to substance.Basic public health services emphasize the "bottom line" in the field of health,focusing on the most basic and realistic public health issues related to national health,and ensuring the "bottom line equity" of society.In modern society,to realize citizens' health rights and interests,to prevent and reduce the health risks of the whole society,has become the basic function of the modern country and an important field of government public service.In the new round of medical and health system reform,the equalization of basic public health services is one of the main goals.The health policies issued by the central and local governments are closely focused on this theme,aiming to achieve equity and justice in the field of health.In order to understand the specific situation of equalization of basic public health services in Chongqing in recent years,this paper systematically calculates the regional differences of basic public health services in Chongqing from the perspectives of time,region,population,geography and economy by using Gini coefficient and concentration index from the development of health services and the practice of health policies in Chongqing.This paper analyzes the problems and difficulties in the implementation of equalization from four aspects: health institutions,health personnel,the amount of diagnosis and treatment at the grass-roots level,and health expenditure.Finally,on the basis of the research results,combined with the theoretical perspective and pilot experience of some developed regions,the paper puts forward some thoughts and suggestions.It is found that the total number of primary medical and health institutions and primary health workers in Chongqing is insufficient,and there is a large gap between Chongqing and developed provinces.There are some differences in the distribution of resources among regions.The health workers are mainly concentrated in the main urban areas and other economically developed areas.The grass-roots health workers in the surrounding districts and counties are relatively short,and the overall professional quality of the staff is insufficient.The accessibility of health services is relatively poor compared with the overall level of the whole country and the level of the western region,and the regional differences are also relatively significant.The southeast and northeast areas of Chongqing are significantly lower than the main urban areas.As a whole,it has not formed a mature social medical pattern,resulting in relatively heavy government and personal medical and health expenditure.On this basis,five suggestions are put forward,which need each region to carry out characteristic service package and enrich basic public health service projects based on local demand;improve the service accessibility of residents in remote areas by implementing the family doctor service mode;further strengthen the talent supply in backward areas,improve the quality of grassroots health personnel as a whole;strengthen the support of social medical policies,and optimize In order to ensure the full utilization of medical and health resources at the grass-roots level,we should carry out universal health education to strengthen the public awareness of hierarchical diagnosis and treatment.
