Research On The Development Of Healthy Old-Age Service Of Traditional Chinese Medicine Under The Mode Of Home-Based Community Old-Age Care

Objective: This study is based on the traditional Chinese medicine service in community health service institutions.Starting from both supply and demand,this study focuses on the demand of the elderly in the community of Yongan City for the healthy old-age service of traditional Chinese medicine,and the current supply of the traditional Chinese medicine service in the community health service institutions.It also explores the advantages,disadvantages,opportunities,and threats of the healthy old-age service of traditional Chinese medicine in the community of Yongan City,and it puts forward the development of the healthy old-age service of traditional Chinese medicine under the mode of home-based community old-age care.Policies and suggestions are provided to meet the needs of elderly health care and pension.Methods: Through surveying,we collected data and analyzed it by the quantitative and qualitative methods.(1)For the elderly and the traditional Chinese medicine technicians in the community,we used the questionnaire survey to test the elderly's needs of traditional Chinese medicine health care service.we also examined the understanding and attitude of traditional Chinese medicine technicians to the healthy old-age service of traditional Chinese medicine.(2)For the administrators from the health departments,the director of community health service institutions,and community medical and health technicians,we adopted the in-depth interview.From the policy perspective and the supply point of view,we discussed the development opportunities for the healthy old-age service of traditional Chinese medicine and development dilemma,plans,and proposals.(3)Using descriptive statistics analysis and chi square test analysis,we analyzed the collected data in term of the factors affecting and the basic situation.Additionally,we used SWOT analysis to identify the advantages,disadvantages,opportunities,and threats of developing the healthy old-age service of traditional Chinese medicine.Results: Demanders:(1)More than half of the elderly(56.25%)prefer the home-based care for the aged.The study shows factors like elderly's age,educational level,living conditions and average monthly income have impacted on the choice of old-age pension mode for the elderly(P < 0.05).(2)95.67% of the elderly need the most for old-age care services is traditional Chinese medicine medical care,and 94% of the elderly said they had been experienced traditional Chinese medicine treatment.The study shows factors like elderly's living conditions,daily living care,income sources,average monthly income,whether to see a doctor in the past two weeks,the frequency of receiving traditional Chinese medicine treatment methods,and the tendency of seeking medical treatment have an impact on the treatment methods of traditional Chinese medicine(P < 0.05).(3)The elderly's demands for health care is ranked in the top three according to the importance in the order of prevention of disease(1402 points),dietary guidance(1065 points)and chronic disease nursing(892 points).The study shows factors like elderly's sex,age,educational level,living conditions,daily living care,medical insurance,number of diseases,demand for old-age services,pension mode,the average monthly acceptable health pension fee of traditional Chinese medicine have impacted on the demand of the healthy old-age service of traditional Chinese medicine(P < 0.05).(4)The elderly recognized the advantages of traditional Chinese medicine,generally believed that traditional Chinese medicine had high safety and few side effects(63.46%),low price(48.32%),long history and trustworthy(45.67%)and good therapeutic effect(29.57%).(5)Most of the elderly(77.99%)are satisfied with the overall evaluation of traditional Chinese medicine services in community health service institutions.However,the elderly generally believe that there are some problems in the development of traditional Chinese medicine health care services,such as lack of relevant professionals(70.43%),imperfect basic hardware facilities(69.95%)and less service items(56.49%).Providers:(1)The community health service institutions in Yongan City have the advantages of wide coverage,reasonable distribution,and convenient transportation.90.48% of the community health service institutions can provide traditional Chinese medicine services.100% of the community health technicians have worked for more than 10 years.They have rich experience in clinical work.96.15% of the community health technicians support the development of the healthy old-age service of traditional Chinese medicine for the aged and are willing to provide related services.Moreover,the majority of the elderly(77.99%)are satisfied with the traditional Chinese medicine services provided in community health service institutions.(2)The ability to provide traditional Chinese medicine service in community health service institutions is generally weak.It mainly manifests in the lack of traditional Chinese medicine diagnostic and therapeutic equipment,low frequency of use,high idle rate,fewer traditional Chinese medicine service items,fewer traditional Chinese medicine health technicians and low professional titles,and the unbalanced development of traditional Chinese medicine service in various institutions.There are problems such as lack of funds,the vacancy of medical insurance for urban and rural residents,the single way of propaganda and low frequency.Conclusion: Home-based community old-age care conforms to the concept of the elderly in Yongan City community.It is conducive to the integration of resources,save the costs of pension,and has strong service convenience,trust and supervision.Traditional Chinese medicine has strong pertinence,excellent curative effect,less toxic and side effects.The idea of preventing disease of traditional Chinese medicine is in line with the idea of community prevention.The mass foundation of appropriate technology of traditional Chinese medicine is good,and it is easy to master and popularize.But there are imbalances in the demand and supply of community health endowment service of the traditional Chinese medicine in Yongan City.The main problems include few items of traditional Chinese medicine health endowment service,insufficient facilities of diagnosis and treatment,fewer health technicians of traditional Chinese medicine,vacancy of medical insurance and lack of funds.It leads to the status quo that the supply of health endowment service of traditional Chinese medicine cannot meet the demand.There are also some external problems,such as the fragmentation of government management,difficulties in implementing supporting policies,the dominant position of Western medicine in the existing health service system,the lack of trust of residents in community health service institutions,and the tense doctor-patient relationship.Under the current situation of a large number of favorable policies,community health service institutions have developed rapidly,residents' awareness of health care has gradually increased and the demands have increased.In addition,residents' trust in traditional Chinese medicine services has also increased.Thus,we suggest promoting the development from the following aspects.Firstly,the government departments should pay more attention to the construction of community health endowment service of traditional Chinese medicine,and strengthen the integration of multi-resources and supervision.Secondly,the government should enrich the team of community health technicians of traditional Chinese medicine,improve community health technology of traditional Chinese medicine,and stabilize the team.The third is to strengthen the capacity-building of community health endowment services of traditional Chinese medicine.Starting with the basic public health services of the state and the contract services of family doctors,the government is expected to enrich the specific content of health endowment services of traditional Chinese medicine.The fourth is to increase government financial investment and establish multi-cooperative financing channels,enrich the community hardware resources.The fifth is to improve the medical insurance catalogue and explore the establishment of long-term care insurance.The sixth is to speed up the construction of informatization.The seventh is to strengthen the propaganda efforts to enhance the health cultural literacy of the elderly.
