The Research On The Service Model Of “Combination Of Medical And Health Care” For Community Home-based Care For The Aged In Tianjin

The rapid growth of the elderly population and the phenomenon of aging,empty nesting and morbidity are becoming more and more serious.In order to actively respond to the aging of the population,we have carried out continuous exploration and attempt.From the pension mode,we have experienced the development of family pension,institutional pension and community home pension;from the service content,we not only aim to meet the basic material needs of the elderly,but also to meet the multi-level service needs of the combination of medical and nursing;from the pension concept,we have changed to the aspects of healthy aging and medical and nursing integration.Through the test of practice,the service mode of "combining medical care with nursing care" for the aged at home in community is more in line with and suitable for the medical care needs of the elderly.Based on Maslow's demand theory,welfare pluralism theory and social embeddedness theory,this paper takes Tianjin community home-based pension service model as the topic,and through questionnaire survey and interviews with community health service center staff,deeply understands the characteristics of Tianjin community home-based pension service model.It is the Contractual service of the combination of traditional Chinese and Western medicine and family doctor.Through the analysis of the survey results,it is found that there are some problems in practice,such as asymmetric access to information between policy and grassroots people,shortage of professional service personnel,imperfect supervision and management system,and unbalanced allocation of health care resources.The reasons for the problems are insufficient policy publicity,insufficient awareness and participation of residents,insufficient training of professional personnel,lack of systematic evaluation and supervision mechanism,and the strength and coverage of community construction need to be improved.According to the problems and the causes of the problems,the paper puts forward the suggestions and Countermeasures of giving full play to the propaganda function to enable grassroots people to obtain comprehensive and accurate information,speeding up the training of professional medical service personnel,improving the corresponding monitoring and evaluation system,doing well in the implementation work,strengthening community construction and expanding the coverage.Through the research on the service mode of "combination of medical care and maintenance" for the aged at home in Tianjin and the analysis of the situation of population aging,the service mode of "combination of medical care and maintenance" for the aged athome in Tianjin has great development prospects and research value.It can meet the needs of different types of elderly people for diversified and diversified old-age services,and provide more comfortable,convenient and healthy for the elderly.Old-age care and medical environment.
