| Objective:Understand the current status of community home care services in Xi'an,analyze the main influencing factors of the elderly 's willingness to provide integrated medical care services,and discuss the needs and problems of the elderly 's home care services for integrated medical care and care in the community Reference data and theoretical basis.Method:This article is guided by the theory of welfare pluralism and Maslow's hierarchy of needs,and refers to relevant literature.From October 2018 to June 2019,Using a convenient sampling method,a questionnaire survey was conducted on 460 elderly people aged 60 years and over in Xi'an,Shaanxi Province.,to conduct a questionnaire survey on the needs of home care services in the community of integrated medical care.Then based on the results of the questionnaire survey,through a semi-structured interview method,interviewed 14 research subjects including the elderly,staff of community health institutions,caregivers of community medical and nursing institutions,and heads of community home care institutions to understand the elderly The content of the demand for home care services in the integrated community of medical care.Results:1.Investigation and research on the integration of medical and nursing care in the community home care service in Xi'an:(1)The age of community care home care elderly in Xi'an is younger,with only 15% of the elderly aged 80 and over;The elderly have a heavy disease burden,with 77.6% of 2 and more chronic diseases.(2)The elderly's satisfaction with community home care services are: living care,spiritual comfort,medical care,and legal services.(3)The elderly's demand for community home care services: day care(80.85%)and elderly table(58.87%)are the main needs of elderly care services;health lectures(78%)and health care(81.78%)are for elderly care The main demand for health care services;entertainment and fitness(89.90%)is the main demand for mental comfort for the elderly;fraud prevention(94.29%)is the main demand for legal services for the elderly.(4)The willingness of the elderly to integrate medical and nursing services: The elderly have a low awareness rate of medical and nursing integrated services.They do not know or have only heard about 63.9%.However,the acceptance of integrated medical and maintenance services is high,and 63.7% are willing to choose integrated medical and maintenance services.(5)After binary classification logistic regression analysis,11 variables such as age,marital status,personal monthly income,and living style are the main influencing factors of the willingness to integrate medical care and nursing services.(6)Among the 293 elderly people who are willing to choose integrated medical and nursing services,after multi-factor logistic regression analysis,age,number of chronic diseases and health self-assessment are willing to choose the integrated medical and nursing services.Main influencing factors;age,number of chronic diseases,self-assessment of health,and self-care ability are the main influencing factors of willingness to choose the integrated medical and nursing service among the types of integrated medical and nursing services.2.A qualitative study on the demand for home care services in the integrated community of medical care in Xi'an:(1)The needs of the elderly for the content of home care services in the integrated community of medical care: the urgent need for professional medical care teams to provide on-site diagnosis and mental support to assist the elderly.(2)Lack of resources for the integration of home care services in the integrated medical care and maintenance community: lack of home care providers in the integrated medical care and maintenance community,lack of professional capabilities of the providers,and lack of supporting equipment for the elderly.(3)home care services for integrated home care services Raise resources and improve the quality of service content.Conclusions:1.The life expectancy of the elderly in the community of Xi'an is equal,the economic situation is good,and the incidence of chronic diseases is increasing.2.The elderly are less satisfied with community home care services,and life care is the most satisfactory service.Services such as day care,on-site services,medical care,health lectures,entertainment and fitness,fraud prevention,etc.are more urgently needed.3.There are many factors influencing the willingness of integrated medical and nursing services.Among them,the elderly have high expectations for family-based integrated medical and nursing services,and they are willing to choose the type of integrated medical and nursing services.Number,health self-assessment,willingness to choose integrated medical and nursing services.Institutional integrated medical and nursing services are affected by age,number of chronic diseases,health self-assessment,and self-care ability.4.Xi'an community professional medical team is extremely lacking,the service content is single,the home care service staff of the integrated medical care and maintenance community is inadequate,and the medical facilities are not fully equipped. |
