As a worldwide epidemic,diabetes has a very low cure rate.Once suffering from this disease,patients need to be treated for life.However,in the course of receiving treatment,the following problems have also been revealed:First,diabetic patients do not control blood glucose very well when they receive a single drug treatment,and the condition is always repeated;secondly,during the course of diabetes treatment Concomitant complications can cause death;Third,older people living alone have higher recurrence and complications than other elderly people.Related studies have shown that a good self-management awareness can make diabetic patients' blood glucose well controlled,so as to achieve better management of the disease and avoid recurrence of the disease.Therefore,this study attempts to apply social work methods to the cultivation of self-management behaviors of elderly individuals living with diabetes alone.The purpose is to study the role of social work methods in correcting the self-management awareness of single elderly patients living with diabetes and to strengthen the elderly living alone with diabetes.The entire community understands and recognizes social work in order to gain more practical experience for social work to get involved in this area and provide services.In order to understand the problems in the self-management level of elderly patients with diabetes in the K community in Nanjing,the researchers measured the self-management level of 30 elderly living with senile diabetes in the K community and found that the unreliable diet structure is common in older individuals living with diabetes alone.,non-regular blood glucose monitoring,neglect of foot care and other issues,which also reflects the K community of elderly living with diabetes alone in general there is a low level of self-management.Therefore,the researchers conducted detailed surveys and visits to the K community,collected detailed data,and then selected eight elderly individuals living alone in the community as subjects for this study,and through scale measurement,participatory observation,and semi-structured interviews.Ways to conduct more in-depth data collection to conduct group activities.Under the guidance of cognitive behavior theory and task center theory,group development mode was used to carry out seven group activities.These group activities were divided into three stages:The first stage was to change the poor cognitive status of diabetes in elderly patients living alone.To pave the way for the next correction of their sense of self-management;the second phase reinforces the self-management behavior of elderly individuals living alone with diabetes through different dimensions,helping them to establish a correct sense of self-management,so as to effectively control the conditions of diabetes.The third phase consolidates the previously established self-management awareness through group activities and strives to allow the research subjects to maintain a good level of self-management after the group activities.Finally,through the process assessment and effectiveness evaluation of the group's involvement,it is found that the self-management level of the group members has been significantly improved,indicating that the social work involved in the self-management of elderly elderly patients with diabetes is feasible.Finally,this study summarizes and reflects the intervention effects of the group's work,and hopes that this study can provide practical experience and lessons for social work in the elderly living alone in the field of diabetes.
