The Research On Elder's Perference Of Medical-nursing Combined Care Mode And Its Affecting Factors

The problem of how to provide for the aged is prominent today.Most old-age care agencies can only provide services for the ordinary elderly in daily life.However,when facing with the multiple needs of medical treatment and nursing for the elderly with advanced age,disability,dementia and chronic diseases,ordinary old-age care agencies cannot provide professional medical services and nursing for this part of the elderly.The separation of "medical care" and "nursing " hinders the healthy and sustainable development of China's pension service industry.In 2013,the state council proposed to promote the development of medical-nursing combined care mode.As for government,it is urgent to solve which medical nursing combined care mode should be implemented for middle-aged and elderly people with different needs in different regions.There are few studies on the influencing factors of medical and nursing combination preference in domestic academic circles,the existing research cannot meet the needs of China's pension status and policy practice.Therefore,analyzing the selection preference of different types of combination of medical care and nursing for the elderly and its influencing factors can not only enrich the theoretical empirical research related to the combination of medical care and nursing,but also provide theoretical support for the future development of medical and nursing care in Chengdu,which has certain guiding significance.On the basis of reading a large number of relevant literatures,I take the middle-aged and elderly people in Chengdu as the research object.Besides,a theoretical model of the selection preference and influencing factors of different types of combination of medical care and nursing for the middle-aged and elderly in Chengdu was constructed,and corresponding questionnaires and interview questionnaires were prepared.Firstly,60 middle-aged and elderly people were randomly selected in the five major urban areas of Chengdu for preliminary survey,reliability and validity test of the scale.Then formal questionnaires were issued and collected.SPSS22.0 software was used to analyze the influence factors of the elderly in Chengdu on the choice of different types of combined medical care and nursing by using the method of binary logistic regression.The survey results show that:(1)Health perception and the willingness of family caregivers have a significant impact on the tendency to select the original medical and health institutions to carry out nursing.(2)Enabling,health perception,understanding of the combination of medical care and nursing care,and community resources have a significant impact on the tendency to select the original nursing care institutions to add medical service qualification.(3)Enabling,pension concept and community resources have a significant impact on the tendency to choose the cooperative mode of combining medical care with old-age care by agreement between medical institutions and old-age care institutions.(4)Enabling,understanding of the combination of medical care and nursing care,the concept of pension,and the willingness of family caregivers have a significant impact on the tendency to choose the integration of medical care and nursing care into the community and into the family.The results of the interview questionnaire show that:(1)the factors influencing the selection of the original medical and health institutions for elderly care services are mainly concentrated in four aspects: medical conditions,physical health status of the elderly,functions of nursing homes and family factors.(2)the factors influencing the addition of medical service qualification in the original pension service institutions mainly focus on the four aspects of physical health,economic level,family factors and service quality.(3)the influencing factors for selecting the agreement between medical institutions and pension service institutions are mainly concentrated in the three aspects of medical level,pension level and social advantages.(4)the factors influencing the selection of integrating medical care and nursing into community and family mainly focus on three aspects: health status,family and social resources,and the concept of providing for the aged.
