The Correlation Between Serum Uric Acid Level And Atrioventricular Block And Elevated Pacing Threshold

Objective:To investigate the correlation between serum uric acid(SUA)level and atrioventricular block(AVB)and elevated pacing threshold in III-degree AVB patients.Methods:64 consecutive AVB inpatients from the Department of Cardiovascular Medicine,First Affiliated Hospital of Chengdu Medical College from Sep.2017 to Sep.2018 were enrolled into case group,and non-arrhythmic patients admitted at the same time were taken as control group,the correlation between hyperuricemia(HUA)and AVB was analyzed.126 consecutive III-degree AVB inpatients undergoing pacemaker implantation from the Department of Cardiovascular Medicine,First Affiliated Hospital of Chengdu Medical College from May.2015 to Sep.2018 were enrolled,subjects were divided into HUA group and control group according to SUA level,relationship between SUA level and pacing threshold was analyzed.Results:Level of SUA in AVB group was significantly higher than that in non-arrhythmic group(6.70±2.33 vs.5.85±1.46mg/dL,P=0.015),the rate of combined HUA in AVB group was also significantly higher than that in non-arrhythmic group(42.19% vs.20.31%,P=0.008),univariate regression analysis showed HUA was correlated with AVB significantly(OR 2.863,95%CI 1.305~6.279,P=0.009),though after adjustment for multiple potential confounding factors step by step,the result stayed statistically significant(P<0.05).In comparison with AVB subgroups,HUA was not significantly correlated with I-degree or II-degree AVB.In III-degree AVB patients,right ventricular pacing threshold was obviously higher than that in control group(0.93±0.15 vs.0.69±0.14 V,P<0.001),univariate linear regression shows that SUA was significantly correlated with right ventricular pacing threshold(R 0.608,P<0.001),analysis of multiple potential confounding factors showed SUA was independent of other potential confounding factors and significantly correlated with right ventricular pacing threshold(? 0.591,P<0.001).Conclusion:HUA was significantly correlated with AVB independently of other potential confounding factors,but there was no significance between the comparison with I-degree or II-degree AVB subgroup.In III-degree AVB patients,the pacing threshold of right ventricular increased with the increase of SUA level,SUA was significantly correlated with right ventricular pacing threshold independently of other potential confounding factors.
