Construction And Evaluation Of Spiritual Care Scheme For Advanced Cancer Patients

Objective:The spiritual care program for advanced cancer patients was constructed by literature review method,and the effects of spiritual care on spiritual health,anxiety and depression,and quality of life of advanced cancer patients were analyzed.Methods:1.Using literature analysis and previous research results of this topic,we constructed a spiritual care program for advanced cancer patients that is suitable for our culture and national conditions,and revised the program through expert meeting method.2.Stratified sampling to select 100 patients with advanced cancer from February to December 2018 in Oncology Department and Ningyang Ward of Shengjing Hospital of China Medical University.They were randomly divided into experimental group and control group with 50 cases in each group.Before the intervention,the general data and clinical data of the two groups were investigated to determine whether the data of the two groups were comparable.The patients in the control group received the current routine nursing in our hospital.The patients in the experimental group received spiritual nursing intervention for advanced cancer patients while receiving routine nursing.The intervention lasted for five times,30-60 minutes each time.After the intervention,the spiritual health,anxiety and depression symptoms and quality of life of the two groups were evaluated.3.Use Epidata 3.1 to input data,SPSS21 to establish a database,collate and analyze the research data.The main statistical methods used in this study include:the measurement data are expressed by mean±standard deviation(x±s),and t-test such as independent sample t-test and non-reference test.The utilization rate and percentage of counting data were tested by 2 test.The test level was 0.05 on both sides,that is,P<0.05.Results:1.A spiritual care program for patients with advanced cancer has been established,which includes the purpose,mode,intervener,dose,theme,specific content and evaluation index.The themes and specific contents of intervention include introducing spirituality,building trust,relaxing spirituality,gaining comfort,reviewing life and building meaning,four ways of life without regret,life education and gaining dignity,each of which corresponds to four specific implementation steps and objectives.2.After the intervention,the overall spiritual health score of the experimental group(2.75 ± 0.74)was higher than that of the control group(2.59 ± 0.69),and the difference was statistically significant(P<0.05).3.There was no significant difference between the two groups in the relationship between mental health and existence and self-relationship dimension after intervention(P>0.05).4.There were significant differences between the two groups in the relationship between spiritual health and others and between important or sacred things(P<0.05).5.After intervention,95.45%of the patients in the experimental group had no anxiety,which was significantly higher than 60.98%in the control group and 97.73%in the non-depression group,which was significantly higher than 85.37%in the control group(P<0.05).6.After intervention,the overall quality of life score of the experimental group(5.36±1.16)was higher than that of the.control group(4.39±1.36),and the difference was statistically significant(P<0.05).7.There was no significant difference in body function,pain,dyspnea,insomnia and constipation in quality of life(P>0.05).8.There were significant differences in emotional function,fatigue,nausea and vomiting,and overall quality of life(P<0.05).Conclusion:The spiritual care program for patients with advanced cancer developed in this study can improve the spiritual health and quality of life of patients with advanced cancer,reduce negative emotions such as anxiety and depression,and this method is simple and feasible.It is suggested that patients with advanced cancer should be widely used clinically in cancer and hospice wards to improve the quality of life and nursing of patients with advanced cancer.
