| ObjectiveTo describe the status quo of spiritual health,life meaning and quality of life in patients with advanced lung cancer,to explore the relationship between them,and analyze the spiritual health related needs and influencing factors of patients with advanced lung cancer.To provide a empirical evidence for improving the spiritual health,life meaning and quality of life in patients with advanced lung cancer.And to lay a theoretical foundation for subsequent research on spiritual health.MethodsThis study adopts a mixed research method and is mainly divided into two phases:(1)A cross-sectional survey was conducted to investigate 324 patients with advanced lung cancer who were hospitalized between January 2017 and May 2017.The questionnaires included: the general information questionnaire,the functional assessment of cancer therapy-general(FACT-G),the functional assessment of chronic illness therapy-spiritual-12(FACIT-Sp-12)and meaning in life scale for advanced cancer patients(Mi LS).A total of 340 questionnaires were distributed and 324 qualified questionnaires were returned with an effective rate of 95%.(2)Using qualitative research methods,based on the results of the first phase,on the basis of combining literature analysis and consulting experts,through semi-structured interviews,using Colaizzi’s method to analyze data to explore the related needs of patients with advanced lung cancer,and further interpret and improve influencing factors of spiritual health.Results1.Cross-sectional survey results(1)The patients with advanced lung cancer have a spiritual health score of 30.36±8.74.The peace dimension score is 9.90±3.00,the significance dimension score is 9.98±2.96,and the belief dimension score is 10.48±4.11.After multiple stepwise regression analysis,the education level,religious beliefs,marital status,and combined chronic conditions entered the regression equation.(2)The meaning of life score is 96.22±11.87,the score of seeking the will of meaning is 14.05±2.79,the score of existential frustration is 17.00±2.84,the score of meaning and satisfaction of life is 13.98±2.53,and the score of life control is 24.97±4.82,the score of bearing suffering is 13.23±2.47,and the score of death acceptance is 12.99±2.94.After multiple stepwise regression analysis,the education level,religious beliefs,marital status,average family income,and chronic disease status entered the regression equation.(3)The total quality of life is 68.79±14.89.The physical status scores is 19.41±5.71,the family social status score is 18.99±6.01,the psychological status score is 16.62±5.01,the functional status score is 13.66±6.58,and after multiple stepwise regression analysis,the education level,marital status,the average family income and the condition of chronic diseases entered the regression equation.(4)There was a positive correlation between spiritual health and life meaning in patients with advanced lung cancer(r=0.447).In the relationships among other dimensions,there was no statistically significant difference in the relationship between the dimensions of death acceptance and spiritual health and their dimensions(P >0.05),all other items are positively correlated;spiritual health and quality of life are positively correlated(r=0.559),and the correlation between the various dimensions is statistically significant(P<0.05),and both are positive correlation;There is a positive correlation between the life meaning and quality of life(r = 0.566).2.Qualitative research results(1)The spiritual health related needs of patients with advanced lung cancer include five major themes: the need for disease treatment,the need for contact,the need for peace,the need for meaning/objectives,and the need for transcendence;the twelve sub-themes: the need for love and being loved,communication and accompanied needs,respected and respected needs,inner peace needs,balanced and unbalanced needs,desired needs,self-fulfilling needs,functional role needs,reinterpretation of life needs,spiritual support needs,religious demand,need to accept death.(2)The factors influencing the spiritual health of patients with advanced lung cancer include not only religious beliefs,marital status,and chronic disease conditions,but also gender,age,cognition of diseases,family and social support.Conclusion(1)Patients with advanced lung cancer have lower levels of spiritual health,life meaning,and quality of life.Medical workers need enough attention.(2)There are many factors that affect the spiritual health,meaning of life and quality of life of patients with advanced lung cancer.The level of education,marital status,and whether or not the chronic diseases are combined are the common influencing factors.The spiritual health and meaning of life of patients with religious beliefs are also relatively high.But the level of family income can affect the patient’s life and quality of life.Therefore,in the spiritual care process,individualized treatment and care should be carried out according to the specific conditions of the patient,actively control the progress of chronic diseases,relieve anxiety and depression,and improve their spiritual health.(3)Patients with advanced lung cancer have a strong demand for spiritual health related needs.Clinical staff should timely assess and meet the relevant needs of patients in order to improve their quality of life. |
