Meaningful Therapy Investigation And Intervention Study On Spiritual Care Needs Of Advanced Cancer Patients

Objective The aim is to understand the current situation and influencing factors of advanced cancer patients’ spiritual needs,and explore the psychological intervention effect to spiritual intervention,based on meaning therapy on advanced cancer patients.Besides,it provides basis for psychological intervention of advanced cancer patients.Methods(1)Using basic information of advanced cancer patients,NSTS,FACIT-Sp and life meaning scale of advanced cancer patients as research tools,426 cancer patients who received treatment from 2 hospitals in Hunan province were investigated by questionnaires,in order to understand the current situation of patients’ spiritual needs and its influencing factors.(2)Using random sampling method,we choose advanced cancer patients who are included into standard and these patient have moderate or higher scores on nurses’ spiritual care needs scale,from department of Oncology,A Third-Class General Hospital of Hunan Province,during February 2018 to May 2018.We divided samples by random number table method in separate wards,just means dividing in two wards of the oncology department by numbering units.For example,10 wards(30 patients)in the tumor-patient ward were included in the control group,and 11 wards(32 patients)were included in the intervention group.Besides,10 wards(31 patients)in the second ward of the tumor were included in the control group,and 11 wards(33 patients)were included in the intervention group.Finally,58 patients in the control group and 60 patients in the intervention group participated the study.Patients in the control group were given conventional physical and psychological care measures,such as mobilizing the subjective initiative of the patient to fight against the disease,relieving the patient’s negative emotions through emotional orientation,mobilizing family members to care for and support patients,and creating a good treatment environment.On the basis of the control group,the meaningful therapy-based spiritual care was given to the intervention group.Finally,we measure the quality of life and spiritual status of the two groups.(3)Data processing analysis was performed using the SPSS 18.0 software package.Description statistics were calculated for mean,forstandard deviation,percentage,t-test,Mann-Whitney University,testchi-square test and multiple linear regression.Spearman correlation analysis of the correlation between spiritual status and spiritual needs,quality of life and sense of life in advanced cancer patients.Results(1)The average level of advanced cancer patients’ needs of spiritual care from nurses are(31.43±4.35)points,including 21 low demand cases (4.93%),344 moderate demand cases(80.75%)and 61 high demand cases(14.32%).(2)Female patients’ needs are obviously higher than male patients’.The patients,below 60 years old,are obviously higher than patients over 60 years old.The patients,with religious beliefs,are obviously higher than that of non-religious beliefs.After spiritual intervention,based on meaning therapy,multivariate linear regression analysis showed that gender,age and religious beliefs were the influencing factors for the spiritual care needs of advanced cancer patients.(3)Before intervention,spearman the results of correlation analysis show that the peace dimension is positively correlated with the NSTS scale in sharing self-sense,helping to think,and creating a good atmosphere dimension(r=0.305~0.417,P<0.01),but not related to exploring spirit and exploring religious practice.(r = 0.995 ~ 0.180,P > 0.05).The meaning dimension,the belief dimension and the NSTS scale share positive perception,help thinking,create a good atmosphere dimension,explore spirit and help religious practice(r=0.212~0.498,P<0.05).Peace and Dimensions,Meaning Dimensions,Faith Dimensions,and Physiological Conditions,Social and Family Conditions,Emotional Status,Functional Status Dimensions,and the Will,Frustration,Meaning and Satisfaction of Life in the FACT-G Scale The life control,suffering tolerance,and death acceptance dimensions were positively correlated(r = 0.254 to 0.561,P < 0.001).(4)After the interventional therapy led to the spiritual care intervention,the scores of the physiological and emotional dimensions of the intervention group were significantly lower than those before the intervention and the control group.In the intervention group,the scores of peace,meaning,belief,social and family status,functional status,will to seek meaning,frustration,meaning of life and satisfaction,life control,suffering tolerance and death acceptance were significantly higher than those before intervention.After intervention in the control group(P<0.05).Conclusions(1)Advanced cancer patients’ needs of spiritual care from nurses are medium,and gender,age,religious belief were the influencing factors of patients’ spiritual needs.(2)Spiritual care intervention,based on meaning therapy,is helpful to improve patients’ live quality and spiritual healthy level.
