Objective:To understand the current situation of the elderly in Datong city,the current situation of the development of the Medical-Nursing Combined,the elder's awareness of the Medical-Nursing Combined,the demand situation and its influencing factors,and to explore the selection tendency of the Medical-Nursing Combined mode for the elderly in Datong city,the demand situation of the service items,and the willingness to pay,etc.,so as to put forward suggestions for the Datong municipal government to promote the development of the Medical-Nursing Combined mode,and to develop it for the The Medical-Nursing Combined provide a reference basis.Methods:Using the method of encounter sampling,two communities were selected in each of the four districts of Datong City,including a higher-level and a lower-level community,and each community encountered a sufficient face-to-face survey of the elderly who met the inclusion criteria.Select 50 valid questionnaires.The survey tool is a self-made questionnaire for the combination of medical and nursing needs,including the basic demographics of the elderly,the health status and medical burden of the elderly,the demand for medical care and the willingness to pay for the elderly,and the demand for service projects.The recovered data was built using EpiData 3.1 and analyzed using SPSS20.0.The data usage rate and composition ratio are described.The single factor analysis uses the chi-square test method and the multi-factor analysis uses the binary logistic regression analysis method.Results:1.Basic situation: A total of 450 questionnaires were distributed,418 were returned,and 400 valid questionnaires were selected after the invalid questionnaires were eliminated.The effective questionnaire recovery rate was 88.9%.Among them,the maximum number of elderly people aged 60 to 70 is 64.5%,accounting for 64.5%;210 males and 190 females,the ratio of male to female is 1.1:1;the elderly with high school education and below account for more than 50%;15% of the elderly;40% of the elderly are currently engaged in mental work in enterprises and institutions before retirement,have stable retirement wages,60% of the elderly who do not have stable income for physical labor;the elderly with only one or two children 50% of the number.2.Health status of the elderly: There are 207 elderly people with good health,193 with poor health or very poor,the prevalence of chronic diseases is 65.5%,and the disability rate is as high as 13%.3.Demand and and Expected Services of Medical Care for the Elderly: 50% of the elderly have the need for medical care and old-age care services;among the elderly who are willing to participate in the combination of medical care,43.6% of the elderly choose the community-based pension model,23.75% choose the hospital's pension system model,and 23.25% choose the pension There are medical department models in the institution,9.4% choose the mode of cooperation between hospitals and old-age institutions;the service items expected by the elderly are from basic to high-level(82%),life-care projects(76%),and health guidance(42.1%),rehabilitation care services(32.2%),clinical care services(14.4%),psychological care(11.9%);50% of the elderly are willing to pay between 1,000 yuan and 2,000 yuan.4.Factors affecting the demand for the Medical-Nursing Combined with the elderly:the education level is high school and above(OR=5.98,95% CI: 1.244-28.74),and engaged in mental work(OR=0.287,95% CI: 0.188-0.437),The per capita monthly income is 2,000 yuan and above(OR=8.977,95% CI: 2.771-29.08),the number of children is 1(OR=0.53,95% CI: 0.344-0.815),self-evaluation is in poor health or very Poor(OR=0.303,95% CI: 0.114-0.804),higher convenience of treatment(OR=2.173,95% CI:1.044-4.523)and higher understanding of medical integration(OR=3.816,95%CI:1.388-10.491),these elders have higher demand for medical care and old-age care services.Conclusion:Datong City has a low recognition of the Medical-Nursing Combined for the elderly,and the demand for Medical-Nursing Combined is strong.Most of the elderly with Medical-Nursing Combined needs choose the mode of community home care,followed by the old-age department and the old-age care institution in the hospital.The medical department model is the least suitable for the cooperation mode between hospitals and old-age institutions;the elderly are less willing to pay,most of them are 1,000 yuan to2,000 yuan;education level,occupation,monthly income per capita,number of children,health status,convenience of treatment Combining with medical care to understand the factors affecting the needs of the elderly.Relevant departments should consider the influence of these factors on the demand for Medical-Nursing Combined of the elderly when formulating the policy of combining medical care and health,and promote the development of the combination of medical care and old-age care services from the actual situation of this city.
