Objective:To investigate the relationship between hematocrit(HCT),blood pressure(BP)and blood pressure variability(BPV)in patients with essential hypertension for providing new ideas and methods in improving the blood pressure compliance rate of hypertensive patients,reducing blood pressure fluctuation and reducing target organ damage in hypertensive patients.Methods: A total of 1,136 patients who had completed 24-hour ambulatory blood pressure monitoring(24hABPM)were collected from the outpatient/inpatient department of the changsha first hospital(changsha hospital affiliated to nanhua university)from June 2017 to February 2019.817 patients aged between 40 and 85 years were selected,including 380 males and 437 females,with an average HCT value of(40.66±3.49)%.According to the diagnostic criteria and grouping criteria of hypertension,817 patients were divided into non-hypertension group(n=84),grade 1 hypertension group(n= 82),grade 2 hypertension group(n=127)and grade 3 hypertension group(n = 524).The clinical data of four groups were collected,including gender,age,smoking history,drinking history,height(m)and weight(kg)to calculating body mass index(BMI)and so on.The same biochemical instruments was used to determine HCT,hemoglobin(HB)and so on.The correlation between HCT and hypertension grade was analyzed by Ordinal ploytomous Logistic regression analysis.Standard deviation of systolic/diastolic blood pressure(SD)at 24 hours,daytime and night was monitored by a 24 hABPM monitor,and BPV was expressed by SD and morning peak blood pressure.Pearson correlation was used to analyze the correlation between HCT level and BPV index.Multiple linear regression was used to analyze the risk factors of BPV index.Results:(1)Age,BMI,taking medicine of CCB,HCT,UA,LDL-C,24 hmSBP,24hmDBP and 24 hMAP were compared in the four groups,and the differences were statistically significant(all P<0.05).The gender,merger of chronic coronary artery disease(CAD),abnormal blood sugar,ischemic cerebral apoplexy,and taking the medicine of anti-thrombotic,statins,?-blocker,angiotensin converting enzyme inhibitor/angiotensin receptor blocker(ACEI/ARB)and diuretic drug,serum creatinine(Scr),hypersensitive c-reactive protein(hsCRP),homocysteine(Hcy),total cholesterol(TC),triglyceride(TG)and high-density lipoprotein(HDL-C)were not statistically significant(all P>0.05).(2)Pearson correlation showed that HCT was positively correlated with 24hmSBP(r=0.166),24hmDBP(r=0.318),24hMAP(r=0.273),BMI(r=0.871),UA(r=0.331)and LDL-C(r=0.139)(all P<0.05).Ordinal ploytomous Logistic regression analysis demonstrated that HCT was an independent risk factor for the levels of essential hypertension after adjusted for Age,BMI,taking medicine of CCB,UA and LDL-C(95%CI 0.021-0.203)(P<0.05).(3)Pearson correlation analysis was used to analyze the relationship between HCT and BPV index,and it was found that HCT level was positively correlated with 24hSBPSD(r=0.090),24hDBPSD(r=0.099),24hMAPSD(r=0.109),nSBPSD(r=0.110),nDBPSD(r=0.113),nMAPSD(r=0.120)and morning peak blood pressure(r=0.104)(all P< 0.05).Multiple linear regression showed that HCT was positively correlated with nDBPSD(? =0.199)(P < 0.05).HCT and LDL-C were positively correlated with nMAPSD(?were 0.175 and 0.286,respectively)(all P<0.05).Conclusion:(1)HCT is correlated with BP,and HCT may be one of the risk factors for essntial hypertension grade.(2)HCT is correlated with BPV,and HCT is the influence factor of nDBPSD and nMAPSD.nDBPSD and nMAPSD increases with the increase of HCT.
