1.The Reference Range And Associated Factors Of Blood Pressure Variability In Elders 2.Effect Of Amlodipine And Felodipine On Blood Pressure And Blood Pressure Variability In Old Essential Hypertension

Background and Objective Blood pressure variability(BPV)has emerged as an important indicator of risk for cardiovascular events among the old,but the reference range and associated factors of BPV are unclear. The aim of the present study was to explore the reference range of 24hBPV in elders and associated factors of 24hBPV in elders with essential hypertension.Materials and Methods The questionnaire contains items on physical examination,laboratory measurement and 24-h ambulatory blood pressure monitoring.The reference range of 24hBPV was estimated using the percentile method among 679 elderly subjects without essential hypertension.Univariate analyses and multivariate regression analysis were applied to discover associated factors of 24hBPV in 1557 old subjects with essential hypertension.Results The finding of the study showed that 95%reference range of 24hSBPV and 24hDBPV among the normal old were(7.11～18.73)mmHg,(5.07～12.89)mmHg.Associated factors of 24hSBPV in old subjects with essential hypertension included 24hSBP(β=0.052,P＜0.001),amlodipine besylate tablets(β=-0.471,P＜0.01),apolipoprotein A I(β=0.009,P＜0.001),apolipoprotein B₁₀₀(β=0.017,P＜0.01),age(β=0.080,P＜0.001), triglyceride(β=0.199,P＜0.05),gender(β=-0.654,P＜0.01),blood uric acid(β=0.003,P＜0.01);associated factors of 24hDBPV included 24hDBP( =0.045,P＜0.001),nifedipine GITS(β=-0.710,P＜0.001),amlodipine besylate tablet(β=-0.421,P＜0.01),age(β=0.061,P＜0.001),body mass index(β=0.080,P＜0.001),apolipoprotein A I(β=0.004,P＜0.01), apolipoprotein B₁₀₀(β=0.007,P＜0.01).Conclusions 95%reference range of 24hSBPV and 24hDBPV among the normal old were(7.11～18.73)mmHg,(5.07～12.89)mmHg.The probable effective ways to decrease BPV are both to decrease the level of BP and to control hazardous factors of atherosclerosis.Amlodipine can be showed a ideal drug that decreases BPV. Objectives To compare the effect of amlodipine besylate tablets(amlodipine)and felodipine extended release tablets(felodipine) on blood pressure(BP)and blood pressure variability(BPV)in old essential hypertension(EH).Methods One handred and twenty old EH patients were randomized into two groups to receive amlodipine(5mg/d,n=60)or felodipine(5mg/d,n=60) for three months,and the effect of two drugs on BP and BPV was compared.Results Both of two drugs could decrease 24hBP effectively(P＜0.05),but amlodipine significantly concolled BPV than felodipine(P＜0.05).Conclusion Amlodipine is a ideal drug that decrease BP and BPV.
