Objectives:The purpose of this study was to compare the clinical effects of Periplaneta Americana powder combined with Lianzhi alum solution gauze on wound after anal carbuncle operation with Lianzhi alum solution gauze.Clinical data were observed and recorded and statistical analysis was carried out to confirm the efficacy and safety of Blattella Americana powder and Lianzhi alum solution yarn for external use of wound after anal carbuncle operation.Methods: The perianal abscess,which met the diagnostic criteria of traditional Chinese and western medicine,was randomly divided into treatment group(Blattella Americana combined with Lianzhi alum liquid gauze)after one-time perianal abscess radical operation under sacral anesthesia combined with venous general anesthesia,and the patients were randomly divided into treatment group(Blattella Americana powder combined with Zhiyan liquid gauze).Control group(Lianzhi alum liquid gauze).The pain degree,wound secretion,wound edema,granulation tissue growth,wound healing time and clinical curative effect were observed and recorded at different time points after operation.Results: From January 2018 to December 2018,92 inpatients with perianal abscess in Chengdu anal and intestinal Hospital were selected,46 cases in each group.There was no significant difference in baseline data(age,sex,original area)between the two groups before treatment(p > 0.05).(1)Degree of pain: the generalized estimation equation was used to compare the pain score between the treatment group and the control group.The results of the analysis showed:(1)time-to-group interaction: b treatment group * time=-0.061,(2)grouping: group b =-0.139,group p = 0.0726,the overall difference between the two groups was not statistically significant.(3)time: time =-1.196,that is,for each additional grade of observation time,there was no statistical difference between the two groups.(3)time: B time =-1.196,that is,for each additional grade of observation time,Pain scores all decreased by 1.196 points.The rank sum test was used to compare the difference of pain score between the two groups at different observation time.The results showed that the pain score of the treatment group was lower than that of the control group on the 14 th day,and the pain score of the treatment group was lower at the 14 th day.There was no significant difference in the other observation time between the treatment group and the control group.(2)Wound secretion: t The generalized estimation equation was used to compare the wound secretion between the treatment group and the control group.The results showed that:(1)the interaction between time and grouping: b treatment group * time=0.061,p < 0.001,The wound secretion of the treatment group and the control group decreased with time,and the decreasing trend of the control group was larger than that of the treatment group.(2)grouping: group b =-0.009,p = 0.910,There was no significant difference between the two groups.(3)time: B time =-0.739.The score of wound edema decreased by 0.739 points for each grade increase of observation time.Rank sum test was used to compare the difference of wound secretion score between treatment group and control group at different observation time.The results showed that the secretion score of treatment group was higher than that of control group on day 7,day 14,day 21,day 1,day 3.There was no significant difference in the other observation time between the treatment group and the control group.(3)wound edema: The generalized estimation equation was used to compare the scores of wound edema between the treatment group and the control group.The results showed:(1)the interaction between time and grouping: b treatment group * time=-0.137,p < 0.001,The scores of wound edema in the treatment group and the control group decreased with time,and the decreasing trend in the treatment group was larger than that in the control group.(2)grouping: group b = 0.271,p = 0.111,The overall difference between the two groups was not statistically significant.(3)time: B time =-0.548,that is,every additional grade of observation time,wound edema score decreased by 0.548 points;Rank sum test was used to compare the difference of wound edema between treatment group and control group at different observation time.The results showed that the scores of wound edema on day 1,day 3,day 7 in the treatment group were lower than those in the control group,and on the 14 th day,the wound edema scores in the treatment group were lower than those in the control group.There was no significant difference between the treatment group and the control group.(4)The growth of and acestoma: The generalized estimation equation was used to compare the growth scores of wound granulation tissue between the treatment group and the control group.The results showed:(1)the interaction between time and grouping: B treatment group * time=0.043,p=0.428,(2)grouping: group b =-0.326,p = 0.038,the average score of granulation tissue growth in the treatment group was 0.326 lower than that in the control group.(3)time: time =-0.630,When the observation time was increased by one grade,the granulation tissue growth score of the wound decreased by 0.630 points.Rank sum test was used to compare the difference of the wound granulation tissue growth scores between the treatment group and the control group at different observation time.The results showed that the wound granulation tissue growth score of the treatment group was lower than that of the control group on the 7th and 14 th day,and was lower at the 21 st day.There was no significant difference between the two groups.(5)The wound healing time in the treatment group was shorter than that in the control group(24.07 ±3.276 vs 25.462.510,p = 0.045).(6)Clinical efficacy: at 21 days after operation,the total effective rate of the two groups was 100%(the cure rate of the treatment group was 21.74%,the obvious effective rate was 78.26%,the cure rate of the control group was 4.30%,the obvious effective rate was 91.40%,and the effective rate was 4.30%).The clinical effect of the treatment group was better than that of the control group(p < 0.006).But the clinical efficacy of the treatment group was better than that of the control group.Conclusions: After anal carbuncle operation,Blattella Americana powder combined with Lianzhi alum liquid gauze was used to relieve the pain of wound,improve the condition of wound secretion,promote wound wet healing,lighten wound granulation edema and promote the growth of granulation tissue after operation of anal carbuncle.Shorten wound healing time and improve clinical efficacy.
