| Objective To explore the value of point puncture and bloodletting combined with flash cupping in the treatment of acute gouty arthritis,and observe the clinical efficacy of point puncture and bloodletting combined with flash cupping in the treatment of acute gouty arthritis,in order to find a simple,safe and effective method of traditional Chinese medicine treatment.Methods A total of 66 patients with acute gouty arthritis were included in this study.According to the random number table method,they were divided into experimental group(33 cases)treated by pricking and bloodletting combined with flash cupping(once every other day,five days as a course of treatment)and control group(33 cases)treated by oral etocoxib(120 mg,orally,once a day,five days as a course of treatment).Each group received one course of treatment.Before and after treatment,"VAS pain score","symptom and sign score","immediate analgesic effect score" and laboratory indicators(serum uric acid,C-reactive protein,erythrocyte sedimentation rate)were used as therapeutic evaluation and observation indicators.Comparing the two groups of patients before and after the treatment of various evaluation indicators,to evaluate the overall clinical efficacy.Result A total of 66 patients with acute gouty arthritis were included in this study.During the treatment period,2 patients dropped out.Finally,64 patients entered the statistical analysis of medical data,32 in each group.(1)There was no significant difference in gender,age,course of disease,VAS pain score,symptom and signs score and laboratory indicators(UA,CRP,ESR)between the two groups(P > 0.05).(2)After treatment: The immediate analgesic effect score between the two groups was compared,and the difference was statistically significant(P < 0.05).(3)24 hours after treatment: VAS pain score and symptoms and signs score of the two groups decreased compared with before treatment,the difference was statistically significant(P < 0.05);VAS pain score and symptoms and signs score of the two groups decreased before and after treatment,the difference was statistically significant(P < 0.05).(4)After one week of treatment,the VAS pain score and symptom and sign score between and within the two groups were compared,and the difference was statistically significant(P < 0.05).(5)Observation of laboratory indicators before and after treatment: There was no significant difference in the determination of serum uric acid(P > 0.05);there was significant difference in the determination of C-reactive protein and erythrocyte sedimentation rate(P < 0.05).(6)Statistical analysis showed that 12 cases were clinically cured,15 cases were markedly effective,3 cases were valid,2 cases were invalid,and the total actovoty ratio was 93%.Control group,8 cases were clinically recurred,17 cases were markedly effective,3 cases were valid and 4 cases were invalid,and the total actovoty ratio was 87%.The total effective rate of the two groups was significantly different(P< 0.05).Conclusion Puncture bleeding combined with flash cupping can effectively improve the symptoms and signs of acute gouty arthritis such as pain,swelling,dysfunction,and the experimental group(puncture bleeding combined with flash cupping)is better than the control group in immediate analgesic effect score after treatment,VAS pain score 24 hours after treatment and 1 week after treatment,symptoms and signs score(oral etocoxib).In the acute stage of gouty arthritis,the serum uric acid value of the two groups did not decrease significantly before and after treatment,but the C-reactive protein and erythrocyte sedimentation rate decreased significantly,indicating that pricking and bloodletting combined with flash cupping can effectively alleviate the inflammatory reaction of gouty arthritis.In the course of clinical practice,the combination of pricking and bloodletting with flash cupping therapy for acute gouty arthritis has the characteristics of high safety and low side effects,which is worthy of clinical application. |
