Objective:The clinical comparative study of acupoint application in treating IBS-D of spleen-kidney Yang deficiency was conducted to observe the clinical effect of acupoint application in treating IBS-D and the effect on intestinal floraMethods:Fifty patients with IBS-D of spleen-kidney Yang deficiency were randomly divided into control group(25 cases)and treatment group(25 cases). The control group was treated with compound eosinophil-lactobacillus tablets;the treatment group was treated with acupoint application on the treatment scheme of the control group. The course of treatment was four weeks in both groups. Before and after four-weeks treatment, the scores of TCM symptoms about diarrhea, abdominal pain, weakness of waist and knee, loss of appetite, cold limbs, fatigue and the total score of IBS-SSS were observed. Before and after four-weeks treatment, the fecal Escherichia coli,Lactobacillus acidophilus,Bifidobacterium and Streptococcus faccalis in the intestine of patients of two groups were quantitatively detected and compared.SPSS19.0 was used to analyze the data of the two groups, and to compare the clinical efficacy of the two groups and the changes of intestinal flora before and after treatment.Results:(1)TCM symptoms:The total scores of TCM symptoms in both groups were lower than those before treatment, which meant that the symptoms of both groups were significantly improved(P<0.01).And,the score of the treatment group was lower than that of the control group(P<0.05).The treatment group was superior to the control group in improving TCM symptoms such as loose stool, abdominal pain, loss of appetite and cold limbs(P<0.05).The curative effect of the treatment group in treating TCM symptoms, such as lumbar and knee weakness, burnout and fatigue, was similar to the control group(P>0.05).(2)Irritable bowel syndrome symptom severity score:Comparing the total score of IBS-SSS between the two groups before and after treatment, the total score of IBS-SSS of the treatment group and the control group after treatment was lower than before, and the symptoms of the two groups were improved, with significant statistical difference(P<0.01).After four-weeks treatment, the total score of the scale in treatment group was lower than that in control group, which indicated that the treatment group was better(P<0.05).(3)Clinical efficacy:According to the curative effect of TCM syndromes, the total effective rate of treatment group was 92%,while the control group was 80%,which meant the treatment group was better(P<0.05).According to the curative effect of IBS-SSS,the total effective rate of treatment group was 92%,while the control group was 84%,which also meant that the treatment group was better(P<0.05).(4)Intestinal flora:The treatment of treatment group and control group both can improve the intestinal Lactobacillus acido philus in IBS-D patients, and the effect of treatment group is better than that of control group(P<0.01).Compared with before, there was no significant improvement in intestinal bifidobacteria,Streptococcus faecalis and Escherichia coli of control group(P>0.05).After four-weeks treatment, the intestinal bifidobacteria of treatment group increased significantly(P<0.01),and Streptococcus faecal is also increased(P<0.05),but Escherichia coli did not decrease(P>0.05).The mechanism of acupoint application in treating IBS-D with spleen-kidney Yang deficiency may be related to the regulation of intestinal flora, which manifested as promoting the growth of probiotics(5)Adverse reactions:There were no adverse reactions in the course of treatment in both groupsConclusion:Acupoint application is a safe and effective external treatment of IBS-D of spleen-kidney Yang deficiencyIt can effectively alleviate the clinical symptoms and improve the intestinal flora of patients.
