Clinical Observation On Curative Effect Of Jianpi Peiyuan Decoction In Treating Diarrhea-predominant Irritable Bowel Syndrome Of Spleen-kidney Yang Deficiency And Effects On Plasma VIP And SP

1 Objective To observe the clinical curative effect of Jianpi Peiyuan Decoction in treating Diarrhea-predominant Irritable Bowel Syndrome(D-IBS)of spleen-kidney yang deficiency and effects on the level of plasma VIP and SP.2 Methods60 cases of D-IBS of spleen-kidney yang deficiency who met all diagnosis standards,were randomly divided into treatment group and control group according to the order of patients see-doctor time,30 cases per group.The control group was given maleic acid trimebutine pieces and bifidobacterium triple viable capsules,simultaneously the treatment group was given Jianpi Peiyuan Decoction on the basis of the control group treatment.After four weeks' treatment,we viewed the patients' changes before and after treatment,including the clinical symptoms,the TCM symptoms,the integration of IBS Quality of Life Scale(IBS-QOL)and the level of plasma VIP and SP.3 Results1)Comparison of the overall effect of the clinical symptoms:In the treatment group there were 9cases recovered,17 cases evidently effective,3 cases of progress,and1 case invalid,so the total of effective cases was 26 cases;while in the control group there were 2 cases recovered,14 cases evidently effective,8 cases of progress,and 6 case invalid,therefore the total of effective cases was 16 cases.Then by comparing the two groups,the overall effective rate of the clinical symptoms in the treatment group was 86.67%,higher than that of the control group 53.33%,P<0.05.2)Comparison of the TCM symptoms:(1)Comparison of the total integration of the TCM symptoms:By the comparison of before and after treatment,the total integration of the TCM symptoms both in the two groups after treatment was significantly lower than that of before treatment,P<0.01;By comparing the two groups after treatment,the total integration of the TCM symptoms of the treatment group was decreased more significantly than that of the control group,P<0.01.(2)Comparison of the single integration of the TCM symptoms:By the comparison of before and after treatment,the treatment group could improve all the single integration of the TCM symptoms,P<0.05,however the control group just could improve the single integration of diarrhea stool,abdominal pain,abdominal distension,bowel,inappetence and languid,P<0.05,and at the same time the single integration of cold chills limbs and lumbar debility had no improvement,P>0.05;By comparing the two groups after treatment,all the single integration of the TCM symptoms of the treatment group were certainly lower than that of the control group,P<0.05.(3)Comparison of the overall effect of the TCM symptoms:In the treatment group,the overall effective rate of the TCM symptoms was 93.33%,indeed higher than that of the control group 70.00%,P<0.05.3)Comparison of the integration of IBS-QOL:By the comparison of before and after treatment,in the treatment group,the integration of anxiety,behavior disorders,health concerns,the body character and dietary restrictions after treatment were all higher than that of before treatment,P<0.05,while in the control group,the integration of anxiety,behavior disorders,health concerns and dietary restrictions after treatment were higher than that of before treatment,P<0.05;By comparing the two groups after treatment,the treatment group was indeed superior to the control group at the improvement of anxiety,behavior disorders,health concerns,the body character and dietary restrictions,P<0.05.4)Comparison on the level of plasma VIP and SP:By the comparison of before and after treatment,the level of plasma VIP and SP both in the two groups after treatment were all lower than that of before treatment,P<0.05;By comparing the two groups after treatment,the level of plasma VIP and SP of the treatment group was lower than that of the control group,P<0.05.5)In the observation during the treatment,there were no obvious adverse reactions both in two groups.4 Conclusion Treated with Jianpi Peiyuan Decoction combined with western medicine,could effectively improve the clinical symptoms and TCM symptoms,raise the integration of IBSQOL which was in order to improve the patients' quality of life,and also reduce the level of plasma VIP and SP.Therefore Jianpi Peiyuan Decoction was indeed worthy of clinical promotion.
