| Objective:To study the clinical efficacy of Xu’s prescription of smoothing the liver and spleen-strenthening combined with acupoint application on diarrhea predominant irritable bowel syndrome(liver stagnation and spleen deficiency syndrome).By evaluating its clinical application efficiency and practicability,The purpose is to provide more appropriate guidance for clinical treatment of traditional Chinese medicine.Method:A total of 74 inpatients and outpatients from the Jiangsu Provence Hospital of Traditional Chinese Medicine were selected from 2018.11-2020.01.Modern medicine is diagnosed as irritable bowel syndrome(diarrhea type),and TCM syndrome differentiation is liver stagnation and spleen deficiency syndrome.They were randomly divided into 3 groups according to the order of consultation.Which are Chinese medicine decoction(with prescription of smoothing the liver and spleen-strenthening combined with acupoint application)group,acupoint application group,and Chinese medicine&acupoint application group.The course of treatment was 4 weeks(28 days),and the efficacy evaluation point was before,after and 2 weeks(14 days)of treatment.The recurrence rate was evaluated by telephone or outpatient follow-up 4 weeks after the end of the course of treatment.According to the clinical symptom score,SSDPRO scale score,IBS-QOL scale score before and after the treatment,the clinical efficacy of Xu’s prescription of smoothing the liver and spleen-strenthening combined with acupoint application was evaluated.Results:1.Comparison of baseline indicators:Before treatment,the three groups of patients were comparable in terms of gender,age,course of disease,and degree of illness,and the differences were not statistically significant(P>0.05),and can be compared.2.Comparison of clinical curative effect:Yigan Jianpi Recipe(Xu’s prescription of smoothing the liver and strengthen spleen)combined with acupoint application group recovered 2 cases,markedly effective 9 cases,effective 13 cases,invalid 1 case,total effective rate 96.00%;Yigan Jianpi prescription group recovered 2 cases,marked effect 2 cases 19 cases were effective,the total effective rate was 95.83%;2 cases were markedly effective in the acupoint application group,18 cases were effective,5 cases were ineffective,and the total effective rate was 80.00%.The comparison between the three groups showed that the acupoint application group was significantly different from the other two groups(p<0.05),and the efficacy of the combined acupoint application group of Yigan Jianpi Recipe and the group of Yigan Jianpi Recipe was not significantly different(p>0.05).3.Comparison of clinical symptoms:The overall improvement was combined with Yigan Jianpi Recipe combined with acupoint application group>Yigan Jianpi Recipe group>acupoint application group(p<0.05).In terms of main symptoms,in terms of abdominal pain/abdominal discomfort,pain reduction after abdominal pain and diarrhea,the curative effect of Yiganjianpifang group and Yiganjianpifang combined point application group is similar,and the effect is better than that of point application group(p<0.05);In terms of stool traits,the Yigan Jianpi Recipe combined with acupoint application group had the best therapeutic effect among the three groups(p<0.05);in terms of improving stool frequency,the efficacy of the three groups was close(p>0.05).;In terms of secondary symptoms,Yigan Jianpi Decoction group,Yigan Jianpi Decoction combined with acupoint application group had similar therapeutic effects(p>0.05),which were better than acupoint application group(p<0.05);In terms of loss of appetite,Yigan Jianpi Recipe combined with acupoint application group has the best effect,followed by acupoint application group(p<0.05);the three groups have similar efficacy in terms of abdominal distension,fatigue,and incomplete bowel movement.(P>0.05).4.Comparison of TCM spleen and stomach diseases PRO scale(SSDPRO):The Yigan Jianpi Recipe combined with acupoint application group is ahead of the other two groups in the overall dimension,especially for F2 pain and discomfort,F3 digestive function,F4 stool condition,The psychological aspect of F6 was significantly improved(p<0.05).There was no significant difference between the pain and discomfort of F2 and the social improvement of F7 in the Yigan Jianpi Formula group compared with the Yigan Jianpi Formula combined acupoint application group(p>0.05);Acupoint application group is superior to Yigan Jianpi prescription group in F8 medical treatment(p<0.05)5.Comparison of quality of life scale for irritable bowel syndrome(IBS-QOL):Yigan Jianpi Decoction combined with acupoint application group in anxiety(DY),behavior disorder(IN),picky eaters(FA),social function((SR)The best improvement was achieved in the four dimensions(p<0.05).Compared with the acupoint application group,the Yigan Jianpi Recipe group can improve patients’ quality of life in terms of health anxiety(HW)and interpersonal relationship(RL).The group had better body image(BI)than the Yigan Jianpi Recipe group.(p<0.05)6.Comparison of drug reeurrenee rates:The recurrence rates of the three groups:Yigan Jianpi Recipe combined with acupoint application group(16%)<Yigan Jianpi Recipe group(41.7%)<Acupoint application group(72%)7.Comparison of drug safety:Adverse reactions occurred in 2 patients in the acupoint application group.The adverse reaction was localized skin pruritus during application,and the incidence of adverse reactions was 8.0%.One patient had mild adverse reactions,and the same was applied to local itching of the skin.The incidence of adverse reactions was 4.0%.Three patients had mild symptoms.The symptoms disappeared after symptomatic treatment,and the course of study was completed.Conclusion:Xu’s prescription of smoothing the liver and spleen-strenthening has significant clinical effects and also improve clinical efficacy by combining acupoint application.Effectively improve the clinical symptoms of abdominal pain,abnormal mood,and endless stool through combined acupoint application.It can significantly improve digestive function,mental health,behavioral cognition,and interpersonal communication.The quality of life of patients has the characteristics of high safety and reduced clinical relapse rate,which can provide a more effective way for clinical treatment and can promote clinical application. |
