ObjectiveOveractive bladder is one kind of syndrome characterized by acute urinary tract symptoms.The etiology and pathogenesis of this disease are not clearfor the momentand lack of effectivetreatments.At present,the main treatment is the oral M receptor blocker to relievethe symptoms,but there are some side effects and the use of taboo,limiting its further application.Traditional Chinese medicine believes that the disease belongs to the category of abnormal lower urinary tractsymptoms,such as "drenching syndrome","detrimental urination" and "drowning".Nei Jing puts forward the theory that "kidney governs water",and believes that the kidney is closely related to the abnormal lower urinary tractsymptoms,which makes a guiding significance for the treatment of the disease.In order to improve the clinical service andexpand the effective methods for the treatment of overactive bladder,acupoint application combined with oral solifenacin is applied and observed in this study,based on Nei Jing's theory of kidney governing water and the principle of Tonifying the kidney.MethodsThrough inclusion and exclusion criteria for screening qualified subjects,90 qualified patients were brought in the study.According to completely randomized grouping principle,the subjects were divided into group A(n=30),group B(n=30)and group C(n=30).Group A adoptedthe treatment ofacupoint application combined with oral solifenacin;Group B adopted the treatment of oral solifenacin purely;Group C adopted the treatment ofacupoint application purely.The groups were treated for 4 weeks.Shenshuacupoint?Yongquanacupoint?Qihaiacupoint?Mingmenacupointand Taixiacupoint were selected for acupoint applicationwith Daiwenjiu paste,8 hours a day,lasting for 4 weeks inall.Record 3 groups of patients' urine Diaries 3 days before the beginning of treatment and 3 days after the end of the treatment.The frequency of daytime urination,nocturnal urination,urgencyand urgent incontinenceon average 24 hours,the scores ofOABSS,QOLand kidney-qi deficiency symptom were both calculatedand analyzedbefore and after the treatment to evaluate the clinical effect.Results1.Before the treatment course,group A,group B and group C did not differ in Gender,age,disease course,daytime micturition frequency,nocturnal urination frequency,urgency frequency,urgent incontinence frequency,OABSS score,QOL score,or kidney-qi deficiency symptom score(P>0.05).2.After the treatment course,daytime micturition frequency,nocturnal urination frequency,urgency frequency,OABSS score,QOL score,and kidney-qi deficiency symptom score were all less than those before the treatment among the three groups,the above differences were statistically significant(P<0.05).Urgent incontinence frequency was less than that before the treatment among the three groups,but there was no statistical significance(P>0.05).3.There was significant differenceon the difference ofdaytime micturition frequency,nocturnal urination frequency,OABSS score,QOL score,and kidney-qi deficiency symptom score before and after treatment course among the three groups(P<0.05).Group A was better than group B and group C(P<0.05),and no difference between group B and group C(P>0.05).In the way of urgency frequency,Group A and group B were better than group C(P<0.05),and no difference compared Group A and group B(P>0.05).In the way of Urgent incontinence frequency,there was no difference among the three groups(P>0.05).4.On the side of the evaluation of western medicine,group Ahad an effective rate of 87%,while group B had 70% and group C had 60%.Group A was more effective than group B and group C(P<0.05),and there was no between group B and group C(P>0.05).5.On the side of the evaluation of chinese medicine,group Ahad an effective rate of 80%,while group B had 43% and group C had 70%.Group A and group C were more effective than group B(P<0.05),and there was no between group A and group C(P>0.05).ConclusionBased on Nei Jing's theory of kidney governing water and the principle of Tonifying the kidney,acute lower urinary tract symptoms and kidney-qi deficiency symptoms in OAB patients could be improved significantly with the treatment ofacupoint application plusoral solifenacin,better than the treatment ofacupoint application or oral solifenacin purely.Combined external with internaltreatment and intergratedtraditional Chinese with western medicine can provide a better curative effect on overactive bladder.Its mechanism is worthy for further study.
