Objective:To observe the effect of electroacupuncture on the level of sleep and cognition in patients with insomnia after stroke, and to explore the relationship between electroacupuncture for improving sleep and improving cognition.Methods:120 patients with post-stroke insomnia and cognitive impairment met the inclusion criteria were randomly divided into 4 groups:sham acupuncture group, drug control group, acupuncture group, and electroacupuncture group. The enrolled patients were all performed on the basis of routine treatment. In the sham acupuncture group,non-acupoint sites next to EX-HN1 were selected,electro-acupuncture was connected after acupuncture, but electrical stimulation was not performed;the drug control group used surazepam tablets(1 mg once);the acupuncture group was selected. In EX-HN1,the needle was retained for 20 min after acupuncture;the electro-acupuncture group used continuous wave based on the acupuncture group and the frequency was 2Hz;the above 4 groups were treated once a day for 21 days.PSQI,sleep diary,ESS and MoCA were used before and after treatment to assess the patient's sleep status, day time sleepiness, and cognitive impairment, and to determine the sleep-related paramet ers and MoCA after treatment.Table scores were used for correlation analysis.Use S PSS23.0 statistical software to process and analyze the data.Result:1.In terms of improving night-time sleep quality, the effective rate of electro-acu puncture group was 90.00%,the effective rate of drug group was 82.14%,the effective rate of acupuncture group was 75.86%,and the effective rate of sham acupuncture group was 46.43%.The difference was statistically significant(P<0.05),suggesting that the electroacupuncture group has the best effect in improving sleep efficacy; The total sleep quality of the four groups was significantly improved after treatment compared with that before treatment(P<0.05).At the same time, the difference between the electroacupuncture group and each group was statistically significant(P<0.05).The needles were superior to the other three groups in improving nighttime sleep;the TST,SE,and SL groups in the sleep diaries of the four groups were compared, and there was no significant difference between the electroacupuncture group and the drug control group(P>0.05).The difference between the acupuncture group and the sham acupuncture group was statistically significant(P<0.01).There was a significant diffe rence between the other groups(P<0.05),suggesting that the electroacupuncture grou p improved the total sleep time, sleep efficiency, sleep latency The effect is better;2.In improving the daytime function, the four groups were significantly improve dafter treatment compared with before treatment(P<0.05),and the difference between the electroacupuncture group and each group was statistically significant(P<0.01).,suggesting that the electroacupuncture group is superior to the other three groups in improving daytime sleepiness;3.In the improvement of cognitive function, both the electroacupuncture group and the acupuncture group were significantly improved after treatment compared with before treatment(P<0.01).The sham acupuncture group and the drug group had no treatment after treatment.Significant changes, the difference was not statistically significant(P>0.05),while the difference between the electroacupuncture group and each group was statistically significant(P<0.01),suggesting that the electroacupuncture group has a significant improvement in cognitive function; The name and orientation of the electroacupuncture group after treatment were significantly higher than those before treatment(P<0.01).The difference between the electroacupuncture group and the other three groups was statistically significant. The el ectroacupuncture group and the acupuncture group treated the posterior vision space with the ability to perform, attention and calculation.,Language ability, abstract abilit y,delayed recall were significantly higher than before treatment(P<0.01),the difference was statistically significant, attention and computational power, language ability, delayed recall between the two groups, electroacupuncture group and the remaining three groups There were significant differences between the two groups(P<0.05).There was no statistical difference between the electroacupuncture group and the acupuncture group(P>0.05).There was a significant difference(P<0.05),suggesting that the elec troacupuncture group had similar efficacy to the acupuncture group in improving visual space, executive ability, and abstraction ability, and was superior to the other two groups in improving attention and numeracy, language ability, and delay Memories have the best effect;4.In the relationship between electroacupuncture improvement sleep and improving cognition, there was a significant positive correlation between total sleep time difference and MoCA total score difference after electroacupuncture group treatment(P<0.01).The difference in sleep efficiency and MoCA total The scores were positively correlated(P<0.05),suggesting that electro-acupuncture for improving sleep is related to improving cognition.Conclusion:1.Electroacupuncture EX-HN1 has a significant effect on insomnia after stroke, can improve sleep quality, reduce sleep latency, increase total sleep time, and improve sleep efficiency;2.Electroacupuncture EX-HN1 has significant curative effect on cognitive disorders, and can significantly improve naming, attention and calculation, language ability, delayed recall and orientation;3.With the improvement of sleep efficiency and the increase of total sleep time, the patient's cognitive function improved significantly, indicating that electro-acupuncture to improve sleep is related to improving cognition.
