Objective: Functional dyspepsia is a heterogeneous disease,the main symptoms are abdominal pain,upper abdomen burning sensation,early satiety,postprandial fullness discomfort and so on as.Acorrding to the main symptoms,Functional dyspepsia can be classified into 3 types: Epigastric pain syndrome,Postprandial discomfort syndrome and overlap.The pathophysiological mechanism of functional dyspepsia is not fully defined at present,and its correlation with Helicobacter pylori infection is of great concern.Many studies have shown that HP plays an important role in FD.However,as far as current research is concerned,the role of Helicobacter pylori infection in the various subtypes of FD is not conclusive.In this study,the differences in Helicobacter pylori infection among different FD subtypes were compared through questionnaires.To explore the pathophysiological mechanism of Helicobacter pylori infection in various subtypes of FD and whether therapeutic responses to H.pylori eradication differ according subgroups of FD.Methods: Collected from June 2017 to December 2017,424 patients from digestive internal Medicine Outpatient Department of the first affiliated Hospital of China Medical University.Exclusion of 110 case with organic disease.218 cases with FD in accordance with the standards of the Roman ?,including EPS(n=118),PDS(n=67),overlap(n=33).96 cases of asymptomatic patients as control group.The questionnaire was used to investigate the sex,age,main clinical symptoms and Helicobacter pylori infection in patients.Helicobacter pylori test method using C13 breath test.Results: 1.There were 218 patients with FD who met the criteria of Roma ?,including EPS 118 cases,PDS 67 cases,mixed type 33 cases,males 80 case(36.7%),female 138case(63.3%).There were 96 cases in the control group,36 in males(37.5%)and female in 60 cases(62.5%).There was no significant difference in gender between FD Group and control group.The average age of 218 patients with FD was 46.4±11.61 years.The average age of EPS group was 45.19±11.57,PDS was 48.12±11.42 years,mixed group was 47.30±12.20 years.The average age of the control group was 47.70±11.5 years.There was no significant difference in age between FD Group and control Group(p>0.05).2.HP Positive was 124 cases in 218 patients with FD,and HP infection rate was 56.9%.HP infection rate in control group was 41.6%.The infection rate of EPS group was 66.1%,and HP infection was obviously related to EPS patients(P=0.003,OR2.73,95%CI 1.56-4.76).The infection rate of PDS group was 43.3%,mixed group infection rate was 51.5%,but there was no statistically significant difference between PDS(p 0.06,OR value is 0.83,95 CI 0.32-1.02)and mixed type P 0.46,OR value 0.67,95 CI 0.36-1.59 with HP infection.3.A total of four major clinical symptoms were selected to compare the remission of symptoms after eradication of HP-positive patients.The remission rate of four symptoms are: upper abdominal pain 81.8%,upper abdomen burning sensation 62.5%,early satiety 63.2%,postprandial fullness discomfort 58.8%,the above abdominal pain remission rate is highest.Conclusion: 1.HP infection rate of EPS is significantly higher than that of other two subtypes.HP may be involved in the onset of EPS.2.The remission rate of upper abdominal pain symptom after HP eradication treatment is higher than other symptoms.
