Objective: Functional dyspepsia(FD)seriously affects the quality of life of children with FD.This study investigated the effects of Hp infection and its antibody typing on the symptoms,serum gastrointestinal hormones,gastric mucosal inflammation and the expression of inflammatory factors and chemokines in children with FD.The purpose of this study was to explore the role of Hp infection in the pathogenesis of FD and to reveal the sense of Hp.The relationship between infection and the incidence of FD provides more theoretical guidance,and provides more reliable basis for the clinical diagnosis and treatment of FD children.Methods : A total of 100 asymptomatic children aged between 4 and 14 who were diagnosed as functional dyspepsia(FD)were collected from the Department of Pediatrics of the Affiliated Hospital of Qingdao University.A total of 100 asymptomatic children were recruited from the C 13 breath Laboratory for health examination.As a control group,the severity of upper abdominal discomfort was determined and scored,and the Hp infection rates of FD children and healthy children were compared.Children with FD were divided into Hp positive group and Hp negative group according to the results of Hp detection.According to the results of Cag A immunoblotting,the positive group of Hp was divided into Cag A positive group and Cag A negative group.Serum levels of serum 5-HT,Gastin,leptin and ghrelin were detected by ELISA.Then the degree of gastric mucosal inflammation was scored.After extracting RNA,the expression levels of related inflammatory factors and chemokines in gastric mucosa were detected by Q-PCR.Symptom scores and gastric mucosal inflammation were analyzed statistically.Differences in Integral,Gastrointestinal Hormone,Inflammatory Factor and Chemokine Expression.Results:There was no significant difference in sex ratio,age and BMI between FD children and healthy children(P> 0.05),but the infection rate of Hp in FD children(60%)was significantly higher than that in healthy children(15%)(P < 0.05).The gastric mucosa of FD children with negative Hp was generally normal,only a few inflammatory cells were found.There were more inflammatory cells infiltration in gastric mucosa of Cag A positive Hp infection group and Cag A negative Hp infection group.The results of immunohistochemistry showed that with the aggravation of Gastric Mucosal pathological changes,the expression of inflammatory factor IL-17 and chemokine CXCL13 increased.The results of ELISA showed that serum 5-HT and leptin and ghrelin in Hp-negative group were significantly lower than those in Cag A-negative group,and the difference was significant(P < 0.01).Gas level also decreased to a certain extent,but the difference was significant(P < 0.01).There was no significant difference(P > 0.05).The serum levels of Gas and ghrelin in Cag A positive group were significantly higher than those in Cag A negative group.The serum levels of5-HT and leptin in Cag A negative group were significantly lower than those in Cag A negative group(P < 0.01).The results of Q-PCR showed that ghrelin,IL-17,CXCL2 and INF-gamma in gastric tissue of children in Hp negative group were significantly higher than those in Cag A negative group,and significantly lower than those in Cag A negative group.The difference was statistically significant(P < 0.01).The levels of IL-17,CXCL13,CCL2,leptin and INF-gamma in Cag A positive group were significantly higher than those in Cag A negative group,and Ghrelin was significantly lower than those in Cag A negative group,and the difference was statistically significant(P < 0.01).The scores of FD symptoms and gastric mucosal inflammation in Cag A negative group were lowest,while the scores of FD symptoms and gastric mucosal inflammation in Cag A positive group were highest.Conclusion: The incidence of Hp infection in FD children was significantly higher than that in healthy children.The dyspepsia symptoms were closely related to the degree of gastric mucosal inflammation,the expression of serum gastrointestinal hormones,gastric mucosal inflammatory factors and chemokines,and the level of Hp infection and its virulence factor Cag A.Hp infection may be one of the important pathogenic factors leading to functional dyspepsia in children;Hp may affect the secretion of gastrointestinal hormones and local mucosal immune response through the role of Cag A,and then participate in the pathogenesis of FD.
