| Objective: To detection serum 25-hydroxy vitamin D and Omentin-1 levels of type2 diabetic patients with osteoporosis,osteopenia and normal bone mass and analysis the relationship between them with glucose metabolism index and bone mineral density.To explore their role in the occurrence and development of patients with T2 DM and osteoporosis.Methods: A total of 140 with type 2 diabetic patients who were in the department of endocrinology of first affiliated hospital of Zunyi Medical College were collected from Feb,2016 to Dec,2017.All participants were divided into three groups according to WHO diagnostic criteria of osteoporosis: normal bone mass group(A group,45),osteopenia group(B Group,45),osteoporosis group(C group,50).A total of120 healthy examination with age and gender matched were collected from the physical examination center of our hospital who received bone density examination which confirmed that the bone volume was normal and served as normal control group(NC group,120)at the same time.Related biochemical parameters,general clinical data and serum level of 25-hydroxy vitamin D of all cases were recorded.Enzyme linked immunosorbent assay was used to measure the levels of serum omentin-1.All data were analyzed by SPSS19.0 software package.All data conforming to normal distribution were calculated by the form of standard deviation of average number and subtraction(x ±s).The quantitative data were tested by one-way ANOVA.Correlation between omentin-1 and other indicators were analyzed by pearson correlation analysis.Influential factors of bone density were analyzed by multiple regression analysis.The differences were statistically significant(P<0.05).Results:(1)Compared with NC group,the levels of FBG,HbA1 c,FINS,HOMA-IRand LDL-C were increased and the levels of HDL-C were decreased in A,B,and C group.(P<0.05).Compared with A and B group,the course of disease,FINS,HbA1 c,and HOMA-IR were increased and the value of bone mineral density(neck)were decreased significantly in C group(P<0.05).(2)The levels of 25(OH)D and omentin-1in C group were lower significantly than those in NC,A and B group,which decreased from normal bone mass,osteopenia to osteoporosis gradually.[(7.18±2.51)VS(11.46±3.46)VS(16.58±3.61)VS(27.41±4.44)] ng/mL,[(47.56±6.52)VS(54.28±15.16)VS(63.58±16.77)VS(68.24±8.31)ng/mL].(P<0.05)(3)Pearson correlation analysis showed that omentin-1 were negatively correlated with HbA1 C,course,age and HOMA-IR(r was-0.792,-0.641,-0.463,-0.889,P<0.05)and positively correlated with the value of neck bone mineral density,25(OH)D(r was 0.639,0.782,P<0.05).Multivariate stepwise regression analysis showed that Omentin-1,25(OH)D,course of disease,and HbA1 c were independent influencing factors of bone mineral density(neck)in patients with T2 DM and osteoporosis.Conclusion: The levels of 25(OH)D and omentin-1 in T2 DM patients were lower than those in the healthy,which decreased from normal bone mass,bone mass loss to osteoporosis gradually.These findings suggested that low levels of omentin-1 and25(OH)D may be involved in the occurrence of diabetes and osteoporosis. |
