| Objective:Diabetes mellitus is a common endocrine disease,and it has a complex mechanism,which is related to the environment and heredity.And diabetes mellitus is a metabolic disorder,which is caused by impaired insulin secretion and/or action barrier.Chronic complications is often associated with cardiovascular,kidney,eyes and nerve lesions,which can threaten life.With the improvement of living standards,the number of people with diabetes increased significantly.Diabetes mellitus affects the quality of life and become a major public health problem.Therefore,early screening of diabetes is very important.The purpose of this study is to discuss the changes of serum irisin and omentin-1 in patients with newly diagnosed type 2 diabetes mellitus,which provides certain guiding significance for the early diagnosis of diabetes mellitus.Methods:(1)subjects: 60 patients with newly diagnosed T2 DM were selected as the experimental group in the department of endocrinology in our hospital from December2015 to May 2016,and 60 patients of healthy subjects were included as the control group,whose age and gender are matched with the experimental group.(2)clinical data collection: Height,body mass,systolic blood pressure(SBP),diastolic blood pressure(DBP)were recorded;fasting blood glucose(FPG),fasting insulin(FINS),2 hour postprandial blood glucose(2h PG),glycosylated hemoglobin(Hb A1c),blood lipid,and other indicators were measured;serum omentin-1 and irisin levels were determined by enzyme linked immunosorbent assay(ELISA).Results:1.The comparison of two groups of clinical observation indexes: There was no significant difference between the two groups in gender,age,BMI,SBP,DBP level difference(P>0.05).FPG,2h PG,FINS,Hb A1 c,HOMA-IR,TG,TC,LDL-C levels of T2 DM group were significantly higher than those in group NGT(P<0.05 or P<0.01).Omentin-1,irisin,HOMA-beta and HDL-C levels were significantly lower than NGT group(P < 0.05 or P < 0.01).2.The correlation analysis between irisin and omentin-1 with observation indexes: Spearman correlation analysis showed that irisin was negatively correlated with FPG,2h PG,Hb A1 c,FINS,HOMA-IR,LDL-C(sr =-0.183,P=0.046),(sr =-0.182,P=0.047),(sr =-0.242,P=0.008),(sr =-0.186,P=0.041),(sr =-0.248,P=0.006),(sr =-0.199,P=0.029),and was positively correlation with HDL-C(sr =0.261 P=0.004);omentin-1 was negatively correlated with FPG,2h PG,Hb A1 c,FINS,HOMA-IR,LDL-C(sr =-0.233,P=0.010),(sr =-0.310,P=0.001),(sr =-0.415,P=0.000),(sr =-0.266,P=0.003),(sr =-0.271,P=0.003),(sr =-0.239,P=0.009),and was positively correlated with HDL-C(sr =0.270,P=0.003);irisin was positively correlated with omentin-1(sr =0.227,P=0.013).3.Regression analysis: Multiple stepwise regression analysis showed that Hb A1 c was related factor for serum irisin(P<0.01).Hb A1 c and HDL-C were associated with serum omentin-1(P<0.01 or P<0.05),and Logistic regression analysis showed that serum irisin and omentin-1 were protective factors of glucose metabolism(OR:0.522,95% CI:0.293~0.930,P=0.027)、(OR: 0.934,95% CI: 0.891~0.979,P=0.005).Conclusions:1.The levels of serum irisin and omentin-1 in patients with newly diagnosed type 2diabetes mellitus were significantly lower than those in normal glucose tolerance patients,suggesting that they might be related to type 2 diabetes mellitus.2.Serum irisin and omentin-1 were negatively correlated with FPG,2h PG,Hb A1 c,FINS,HOMA-IR,LDL-C,and were positively correlated with HDL-C;irisin was positively correlated with omentin-1,suggesting that irisin and omentin-1 might be involved in insulin resistance and lipid metabolism.And irisin and omentin-1 might exist mutual influence.3.The serum levels of irisin and omentin-1 were protective factors of glucose metabolism.Multiple stepwise regression analysis showed that Hb A1 c was related factor for serum irisin;Hb A1 c and HDL-C were associated with serum omentin-1,suggesting that serum irisin and omentin-1 were affected by glucose and lipid metabolism.And the decrease of irisin and omentin-1 might be one of the reasons for the occurrence and development of diabetes. |
