Prediabetes is a reversible phase of T2 DM.In this phase,an appropriate intervention can significantly prevent or delay the onset of T2 DM.So it is the key period of prevention and control.Unfortunately,in our country,the diabetes prevention and control project,which has been widely carried out in community health services,has not covered the population.Even more difficult is that there is no special symptoms and discomfort in prediabetes.The primary health service worker and patients themselves subjective prevention and control consciousness is weak,do not attach importance to preventive intervention activities,compliance rate is lower.The intervention activities for this population are lack of clear and effective model from macro strategy to key technology.In this study,we first analyzed the research frontiers and hotspot of T2 DM mechanism through literature study,to provide basic evidence for the prevention and control of the prediabetes population,and then,use the qualitative study to establish the task of behavioral intervention and to observe the effectiveness of these interventions and compliance.In order to lay the foundation for the further study of the comprehensive management and control mode of the prediabetes population in the community.The results of the study are as follows:The software CiteSpace ?was used to analyze the development process of pathogenesis,research hotspots and frontiers of T2 DM.T2DM belongs to the class of diseases with chronic inflammatory response.Mainly manifested as insulin resistance with relative or absolute insulin deficiency.high glucose,high fat,obesity,genetic defects,intestinal flora imbalance and other factors can stimulate the production of a large number of cytokine to oxidative stress,affecting the funcation of pancreatic ? cells,reduced the insulin sensitivity and promote the progress of T2 DM.According to the mechanism of T2 DM,the behavioral intervention task of prediabetes population was estabilished by literature research and qualitative study.It includes three themes(exercise,health diet,regimen method of traditional chinese medicine),eight sub-themes.Establishment of a prediabetes intervention cohort in a representative Chongqing community.Among the 258 people with prediabetes,121 were impaired fasting glucose(IFG)(46.90%),137 were impaired glucose tolerance(IGT)(53.10%).Have been take comprehensive measures to promote the implementation of the intervention tasks.One years after,there were 88(34.10%)blood glucose converted to normal,156(60.47%)were prediabetes,14(5.43%)progress to T2 DM.Highly compliant group of intervention had 48(41.38%)blood glucose converted to normal,3(2.59%)progress to diabetes.Moderate compliance group of intervention 34(33.66%)converted to normal,6(5.94%)progress to T2 DM.Poor compliance group 6(14.63%)converted to normal,5(12.20%)progress to T2 DM.It shows the important role of behavioral interventions.Compliance with health behavior directly influence the effect of behavioral interventions.Research results showed that,the higher age groups,retirement status and high health awareness of diabetes were the independent variables factors of increased compliance.to increase the health awareness,the ability of self management,and the interesting team activities,are important methods which worthy of further exploration to improve the community compliance.
