Objective:Compare with the segmental dentate ligation,to observe the clinical curative effect of the minimally invasive surgery of linear incision with dentate ligation on the treatment of stage III and IV varicose annular mixed hemorrhoids.Methods:40 patients with stage III and IV varicose annular mixed hemorrhoids admitted from Nanjing Hospital of Chinese Medicine were selected from May 2017 to January 2018.The patients were divided into two groups,and 20 cases were in each group.The treatment group was treated with a minimally invasive surgery of linear incision with dentate ligation,and the control group was treated with a segmented dentate ligation.The clinical efficacy on postoperative hemorrhage and prolapse,postoperative complications,operative time,and wound healing time,etc.were compared between the two groups and the patients were followed up for 3 months.The differences in efficacy,minimally invasiveness,and complications were analyzed.Results:The total effective rate of the treatment group and the control group was 100%,and there was no significant difference(P>0.05).There was no statistical difference in postoperative prolapse and hemorrhage between the two groups(P>0.05),and the same in postoperative edema and postoperative infections.There were statistical differences between the two groups in terms of operation time,postoperative pain,postoperative urinary conditions,wound healing time and postoperative anal function(P<0.05);there was statistical difference between the two groups in the exudates on the first,third,fifth and seventh days after surgery(P<0.05),but there was no statistical difference in the exudates on the 10th day after surgery(P>0.05).There was no statistical difference in the anal flatness on the 15th day and 3 months after the surgery(P>0.05),but 1 month after the surgery,the anal flatness of the treatment group was slightly worse than that of the control group,with statistical significance(P<0.05).Conclusion:Minimally invasive surgery of linear incision with dentate ligation is effective for stage ? and ? varicose annular mixed hemorrhoids,which balances the relationship between "exhaustive treatment" and "protection of anus function".It has the advantages of less trauma,shorter operative time,faster postoperative recovery,and less postoperative complications.
