Objective: To observe and objectively evaluate the clinical efficacy of multi-v incision and lateral incision for severe circular mixed hemorrhoids.Methods: 60 patients with severe circumferential mixed hemorrhoids were randomly divided into two groups.The treatment group(n=30)was treated with multiple V incision plus lateral resection and control group(n=30)was treated with external exfoliation and internal ligation.Postoperative urinary retention,postoperative bleeding,postoperative anal edema,anal stenosis and healing time were compared.Results:1 Comparison between the two groups of total efficacy(30 days after surgery)(Table 4)The total effective rate of the two groups was 100%.The total curative effect of the two groups after 30 days was tested by rank sum test.The total curative effect was 30 days after surgery,P>0.05,the difference was not statistically significant,indicating that the two groups were postoperative for30 days.There was no significant difference in total efficacy.2 Comparison of postoperative urinary retention,postoperative pain and perianal edema between the two groups(Table 5-7)By comparing the scores of postoperative urinary retention,anal edema and postoperative pain(24 hours and 3 days and 7 days after operation)between the two groups,the difference was statistically significant by rank sum test(P<0.05),suggesting that the treatment group was reducing the incidence of urinary retention.The relief of pain and anal edema 24 hours and3 days after operation was better than that in the control group.3 Comparison of postoperative bleeding and anal stenosis between the two groups(Table 8,Table 9)The scores of postoperative bleeding and anal stenosis in the two groups were compared with each other by rank sum test(P>0.05).There was no significant difference between the treatment group and the control group in postoperative bleeding and anal stenosis.4 Comparison of healing time between two groups(see Table 10)T test showed that the healing time of the treatment group and the control group was significantly lower than that of the control group(P<0.05),indicating that the healing time of the treatment group was less than that of the control group.Conclusions:1 Both surgical methods have achieved satisfactory clinical curative effect in the treatment of annular mixed hemorrhoids,with no recurrence after6 months.2 Compared with external stripping and internal ligation,multi-V incision plus lateral incision can significantly reduce postoperative pain,anal edema and urinary retention.3 Multiple V incision plus lateral incision can shorten wound healing time compared with external stripping and internal ligation.4 There was no significant difference in postoperative bleeding and anal stenosis between the two groups.
