Study On Corporate Governance Structure Reform Of A Public Hospital Of Dongying City,China

Due to historical and institutional reasons,there exist several problems in the most public hospitals,such as unclear property ownership,un-guaranteed investment,lagged-behind internal management,etc.Therefore,the welfare of most public hospitals is weakened.The operation efficiencies have decreased."Difficulty and high expense in seeing a doctor" has become a severe problem,which constrains the development of medicine and health.The traditional public hospital management system cannot meet the needs of the development of medical and health care and the increasing demands of the people.With the development of a new round of medical and health system reform,new requirements of public hospital corporate governance mechanism reform have been put forward.In "Opinions of CPC Central Committee and State Council on Deepening the Reform of Medical Health System",it is clearly put forward to " set up and improve the hospital corporate governance structure,to clear the liability of owners and managers,and to form the mechanism of balancing decision-making,execution and supervision.As a pilot hospital of State Institution Registration Bureau for corporate governance structure construction and the trial reform public hospital in Shandong,sample hospital actively explores management mechanism,operation mechanism and guarantee mechanism according to the institution classification reform requirements and public hospital reform requirements.The hospital tries to construct corporate governance system which suits the practice of hospital development.Since the implementation of the pilot work,the relationship between the government,the Shandong Provincial Hospital and the relevant departments has been preliminarily adjusted.The status of the hospital independent legal person has been established.The goal of the reform of "development of hospitals,encouraging workers and affordable" has been preliminarily achieved,providing valuable experience for the reform of corporate governance structure in public hospitals.Through the case study and empirical analysis of corporate governance structure reform in Dongying People's Hospital at the background of co-operation and co-construction,the thesis explores the operation models and mechanisms of influence of corporate governance structure reform and summarizes the procedures of the reform and their effects.The problems and difficulties in the reform are analyzed.The countermeasures and suggestions are put forwards from five aspects,including improving public hospital management system,improving the public hospital governance system,improving the scientific compensation mechanism,constructing long-term supervision and restraint mechanism,and breaking the boundary of departmental interest conflicts.The above study finds that it is an important guarantee for the welfare of public hospitals to establish public hospital corporate governance structure in which decision-making,management and supervision are separated and mutual constrained.The governance structure,which can improve the operation efficiency and service ability,is an effective way to combine government domination and market mechanism.It is also a requirement of separating functions of government agencies and public institutions and separating management and regulation in the state new medical reform program,an efficient way to construct modern hospital management regulations,to modernize management systems and management ability.By establishing modern corporate governance structure in hospitals,clarifying responsibilities of government,relevant departments and hospitals,and energizing the development of hospitals,the system malpractices in the development of public hospitals can be solved and the healthy sustainable development of public hospitals can be promoted,so that the high-quality,highly-effective,convenient and inexpensive medical and health care services can be provided for the people.
