| Based on the analysis of China’s national governance system and capacity,as well as the public hospital reform,this study raised the following research questions:what structural and procedural problems,restricting realization of good governance,existed in the governance system of Inner Mongolian public hospitals,and what reform measures could be made to solve these problems for good governance?On the basis of the theories of governance,good governance and cooperative governance,through exploring the reality of Inner Mongolia,some core concepts,such as public hospital governance and nonprofit nature of public hospitals,were defined;the goal of realizing good governance was presented for the governance system reform in Inner Mongolian public hospitals under the background of new health care reform and a governance evaluation framework based upon good governance was constructed.Taking the research questions as the guidance and Result-Process-Structure model as the approach,this study analyzed the current situation and problems of public hospital governance in Inner Mongolia from the perspective of good governance.It was found that the good governance degree(degree of nonprofit regression)in Inner Mongolian public hospitals was insufficient,mainly manifesting as:unreasonable structure of medical resource allocation,gaps in medical service capacity,inefficient utilization of medical resource.high medical expenses and heavy medical burden.The reasons for poor governance were poor during governance,mainly including:inharmonious health financing proportion,balance mechanism defieiency in medical resource allocation,guidance insufficiency in assessments,halfway reforms in personnel salary system,low management capacity,artificially high drug prices and inconsistent relation among"Medical Treatment","Medicine" and "Medicare Premiums".And the causes of being poor during governance were poor in governance structure,mainly including:non-separation of regulation from operation,unclear and unscientific powers and responsibilities in internal governance,insufficient participation of social organizations and market and inadequate coordination between the government,society and market.Guided by the causes of problems,this study made suggestions for governance system reform of Inner Mongolian public hospitals.Firstly,suggestions were made for governance structure reform:building a macroscopic governance landscape coordinated by the government,society and market with giving full leadership to the Party Committee and the National People’s Congress(NPC)as meta-governance bodies in collaborative governance,reforming the administrative system to surely separate regulation from operation,developing social organizations and private hospitals and making a clear coordination mechanism between the government,society and market;optimizing internal governance structure to establish the internal governance lead by a constitution and implement the responsibility system of hospital president led by the Party Committee.Secondly,suggestion was made for the governance process reform:making the "Medical Treatment","Medicine" and "Medicare Premiums" coordination mechanism with"Medical Treatment" at the lead. |
