Application Of Enhanced Recovery After Surgery Comfortable Nursing In Patients Undergoing Total Knee Arthroplasty In Perioperation

ObjectComprehensive enhanced recovery after s urgery with comfortable nursing,discuss the effect of enhanced recovery after surgery with comforting care nurs ing on total knee arthroplasty patient,investigat patients pain,recovery of knee function?quality of life and the satisfaction in hospital,and provide clinical experience and scientific basis for joint recovery by enhanced recovery after surgery and comfortable nursing.MethodThrough the methods of literature review,group discussed and preintervention to determine the comprehensive nursing of enhanced recovery after surgery with comfort nursing.Using the convenience sampling methods,select 82 elderly patients in Shandong province Q ingdao hospital of who were admitted to hospital in 2015.9 ~ 2016.4 underwent total knee arthroplasty,we using the general information questionnaire to collect data,baseline assessment of patient,include patient general information and joint functionand pain score.Patients who were eligible for baseline assessment were admitted in the study.41 patients who were admitted at 2015.9.8 ~ 12.8 as the control group(n1=40),and 41 patients who were admitted at 2015.12.10 ~ 2016.4.1 as the observation group(n2=40).Patients in the observation group were treated with ERAS comfort nursing,and the control group was given traditional nursing measures.The two groups were compared before and after the operation by the hospital for special surgery score(HSS score),pain catastrophizing scale(PCS score)and visual analogue scale(VAS score),recorded and compared with postoperative foot activity time,postoperative ambulation time,and average postoperative hospitalization days and sleeping quality.Result1.Two groups of patients in age,sex,body mass index,preoperative underlying diseases,admission HSS scores,VAS score on admission,the basic data of joint activities,preoperative hemoglobin,culture degree,operation time were not statistically significant(P > 0.05).2.The patients in the observation group boleos start time(h)(2.56 ± 1.25)was less than the control group(11.48 ± 4.48)for the first time,postoperative ambulation time average(d)(2.06 ± 3.31)was less than the control group(2.66 ± 0.47),postoperative 12 h VAS score(4.7 ± 1.01)was lower than that of the control group(6.57 ± 1.19),the difference was statistically significant(P < 0.05).3.The after 3d operation HSS score of the observatio n group(34.21 ± 1.65)was higher than the control group(31.23 ± 1.83),and the after 1month operation HSS score of the observation group(83.23 ± 3.81)was significantly higher than the control group(78.69 ± 5.06),the difference was statistically significant(P < 0.05).4.Repeated measures analysis of variance showed that the difference was statistically significant and time effect scores of the two groups in the rang of motion and pain score between the groups(P < 0.05),and the pain score and the time factor of joint activities have interactive effect,the difference was statistically significant(P < 0.05).5.The average length of stay in the observation group(4.48 ± 0.87)was shorter than that the control group(6.81 ± 1.53),and the hospitalization satisfaction(97.71 ± 2.78)was higher than the control group(94.16 ± 6.81),Good sleepig quality in the observation group was higher than the control group,the difference was statistically significant(P < 0.05).Conclusion1.The application of comprehensive enhanced recovery after surgery with comfortable nursing measures for total knee arthroplasty patients of perioperative can shorten the first postoperative time of getting out of bed.increased patient daily exercise,shorten patients' recovery time of joint function,shorten the length of hospital stay.2.The application of comprehensive enhanced recovery after surgery with comfortable nursing measures for total knee arthroplasty patients of perioperative,multimodal analgesia management can effectively relieve patients' pain level,relieve patients' psychological pressure,improve the emotional state of patients,improve patient comfort and satisfaction with physiological.3.The application of comprehensive enhanced recovery after surgery with comfortable nursing measures for total knee arthroplasty patients of perioperative,can accelerate the recovery rapid of joint function after operation.
