Study On The Chronic Disease Management Among The Aging Population In Community Health Service Institutions In Chongqing City

ObjectivesRegarding Chongqing city as empirical research object,through sorting out theory of chronic disease management among aging population in community health service organizations and summarizing relevant experience at home and abroad,this study conducted quantitative research and qualitative research on the status quo of management of the elderly patients with chronic diseases in community health service organizations in Chongqing city.By making comprehensive analysis on human resources,health service efficiency and data,this study explored the feasibility and strategies of chronic disease management among aging population in community health service organizations in Chongqing city,and provided theoretical and practical reference for the research of chronic disease management among aging population in community health service organizations.Data SourceLiterature,research reports and information about chronic disease management among aging population in community health service organizations were collected by searching databases,checking related books and periodicals and retrieving search engine,and related statistical data or materials such as laws and regulations,policy documents and annual statements were from government official websites of health,finance,civil administration and statistics.Empirical materials of each sample areas were acquired through self-designed questionnaires.700 questionnaires were distributed,and there are 675 valid questionnaires.Research MethodLiterature research method,questionnaire survey,Delphi method,statistical analysis,etc… Results12 districts and counties in Chongqing has been successfully created for the national demonstration area of chronic disease prevention and control,there are some achievements in management of chronic disease prevention and control,including family doctor service,chronic disease management has be brought into the annual social career development plan of districts and counties and the residents health education.But there are some problems:(1)There is a shortage of special funds for the prevention and control of chronic diseases and a single source of financing;(2)There is poor allocation of human resources in the community and uneven technical level;(3)There is a lack of cooperation between departments,and the prevention and control system is imperfect;(4)The construction of information system is backward,which is not conducive to the prevention and control of chronic diseases and the sharing of information.Suggestion1.The government should make effective chronic disease management policies,the establishment of standard management system,adjust the structure of fiscal expenditure and increase funding for community elderly chronic disease management.2.Departments should strengthen cooperation and maintain relations of cooperation with scientific research institutions,promoting the transformation of scientific research achievements into practical results,community institutions should focus on chronic disease management of weak elderly in Chongqing.3.To create a healthy living environment and promote a healthy lifestyle,strengthen the self-management awareness of aging population with chronic diseases.
