Research On Elderly Chronic Disease Management In Community Health Service In Shenyang City

The provision of age-related chronic disease management through community health service agencies is receiving more and more attention from the whole society.In recent years,with the continuous expansion of the elderly population in Shenyang,the demand for daily drugs caused by non-hospital treatment and senile diseases is increasing.How to prevent senile diseases,what medicines to use after getting sick,how to eat,how to maintain daily life,and increasingly become The concerns of the elderly.The paper is based on interviews with chronic disease management and services in three community health service centers of the “ National Top 100 Community Health Service Centers ” in Shenyang,with the management of chronic disease management as the entry point,from the perspective of chronic disease management needs and management services.To analyze the current situation and existing problems of chronic disease management in the elderly,and to find that the software and hardware configuration of chronic disease information management is insufficient,the self-management awareness of community residents with chronic diseases is insufficient,the effective services of family doctors are inadequate,and the clinical experience of general practitioners in community health service centers.Insufficient and chronic disease management,multi-subject participation awareness is not strong,is the reason for restricting the improvement of elderly chronic disease management services in community communities.For the management of elderly chronic disease management in excellent community health service centers in other regions of China,through network management services for visiting linkages;referral supervision services for high-risk groups;and preventive intervention services for community chronic disease management,etc.The actual situation of elderly chronic diseases in the community in Shenyang was comprehensively adopted.Finally,the author puts forward the countermeasures to strengthen the management of chronic disease community in the elderly,such as establishing a health record management information platform,improving the special health checkup service for the elderly,expanding the antenatal service of the general practitioner,and improving the qualityof service for chronic diseases.The strengthening of community management for elderly chronic diseases in Shenyang provides a useful reference.
