Objective For the sake of comparing the short-term pain relief and functional improvement of lumbar disc herniation treated with radiofrequency ablation combined with ozone injection and radiofrequency ablation through C arm x-ray.To evaluate the short-term clinical effect of percutaneous target radiofrequency ablation combined with ozone injection in the treatment of LDH.Methods 40 eligible LDH hospitalized patients were included from October 2015 to October 2016,randomly divide into two groups of A and B,each group of 20 cases, group A was treated with targeted radiofrequency ablation combined with ozone injection, group B was treated with radiofrequency ablation alone.Among them: group A of 14 males and 6 females, the average age(39.19±6.87)years old, body mass index(23.28±1.88)Kg/m2,average course of disease(3.94±1.97)months,the incidence of disc herniation is a single segment, among them:L3/4 5 cases,L4/5 12 cases,L5/S1 3 cases.Group B of 12 males,8 females, the average age(38.92±7.13)years old, body mass index(23.36±1.90)Kg/m2,average course of disease(3.68±1.82)months,lumbar disc herniation disease segment L3/4 5 cases,L4/5 11 cases,L5/S1 4 cases. The general data of the two groups of patients were comparable,The difference was not statistically significant(P>0.05).Preoperative examination was performed in both groups with Numerical Rating Scale(NRS),Oswestry Disability Index(ODI)and angle of straight leg raising(SLR).The NRS score,ODI score, straight leg raising test and modified Macnab criterion(the excellent and good rate)were evaluated at immediately after operation,3 days after operation,1 week after operation and 1 month after operation. All cases were performed by the same orthopedic surgeon.SPSS 16.0 statistical software was used to analyze the data. Results All cases were successfully performed, the effective rate of follow-up was 100%.Two groups of patients did not appear intervertebral discitis, end plate inflammation and other complications. There was no statistically significant difference in preoperative NRS score,ODI score and SLR between the two groups(P>0.05).Compared with preoperative, both group A and B after surgery,NRS score and ODI score were decreased,SLR were increased(P<0.05).Comparison between the two groups, the scores of NRS and ODI were significantly lower in group A than those in group B,difference was statistically significant(P<0.05).Patients in group A had higher postoperative SLR than patients in group B,the difference was statistically significant.Postoperative modified Macnab criterion assessment: postoperative immediately,3 days after the difference between the two groups was not statistically significant(P> 0.05),1 week after surgery,1 month after surgery A group was significantly higher than the B group, the difference between the two groups was statistically significant(P <0.05).Conclusion In the accurately grasp of surgical indications and contraindications under the premise, the treatment effect of lumbar disc herniation by targeted radiofrequency ablation combined with ozone injection is better than the use of target radiofrequency ablation.Combined treatment of postoperative recovery time is shorter, safe and reliable, the short-term pain relief and functional improvement is better,,short-term effect is ideal.
