With the arrival of aging society, coronary heart disease, hypertension, diabetes and other chronic diseases become harmful to the health of elderly people.Traditional outpatient service is essential, but it can not work well without the support of patient's own health management.Self health management and medical service provide a new way to control chronic disease.Mobile health stands out as a powerful to control because of its real-time,convenient and efficient features.Mobile health has become the trend of chronic disease management, however,it is still in its infancy.Domestic research in this field focuses on the technical aspect, but lacks empathy of users'.Most foreign scholars deals with the problem from behavior, psychology and management subjects, not compliant with chronic disease, thus it is of limited value to elderly disease management services design.Through literature review and case analysis,we can understand the current development of chronic disease management at home and abroad, then summarize its typical features and patterns, analyze the use of mobile health service for elderly patients, and come up with a set of interaction design principles and service design strategies to influence and change elderly patients' behavior. This paper analyzes the characteristics and needs of elderly patients and analyzes the problems encountered by patients in various stages of self-management,using the method of interdisciplinary research, combined with interactive design, user experience design, psychology and behavioral science study in order to help the elderly manage their own health status,to help them establish a good behavior and change the bad habits of chronic disease management.Improve the existing mobile health apps by eliminating the usage barriers from the perspective of service design, then put forward a behavior model to change the users' behavior and habits and finally come up with a set of service design strategies to better elderly patients' disease management experience.Better other apps usage on the basis of this strategy.
