The Service Design Strategy Research Of Community Based Chronic Disease Monitoring Treatment

Today with the development of information technology, community hospital also do some work about chronic disease health management. This paper researches some current problems of the chronic illness in the self management and community management. Combined with the theory and method of the service design, this paper from the perspective of community, builds the relationship between doctors and patients from two aspects of self management and community management. Improved the level of chronic diseases management, optimized the service system of chronic disease health management.At first, this paper studies on the present situation and the development of chronic disease management, then analysised the the value of community involvement in medical field through the case study. After that did some field investigation, the main work were telephone survey and field survey on patients and their family member. After preliminary user interviews, I used two service design methods issue card and issue map to do some qualitative analysis of the management of the disease and the management their lifestyle.In the deep research I did some deep interview on patients, their family member and community medical staff, to determine the target user and the main scene. Then, through the questionnaire to do some quantitative analysis, explored their demand and acceptance of using mobile in the field of healthcare. Finally it is concluded that feasibility of direction that community involvement in medical field, output community based service system platform strategy and system design.The research results and the main conclusions are as follows. Firstly, the case study identified the theory assumes that community involvement in medical field.Secondly, digged the existing problems of two management ways through the field investigation. In the end, builded the chronic disease management service system based on community. Community organization were mainly for the local community and virtual community. Local community is based the community hospitals as a professional entity to support patients’ health management. General practitioners in hospital and patients has established the positive communication service platform, which can improve the enthusiasm and the cognition of patients to disease. For the hospital, they can connect service ring in the management of chronic diseases, convenient community doctors to give patients some health guidance about management, to improve the chronic diseases management of patients. Virtual community is breakthrough regional restriction, all of the participants are interest oriented to participate in the community. To built community on the basis of patients’ collaboration to promote experience sharing and lifestyle. In the whole service system for management of chronic illness, through the way of community based to promote the development of chronic disease management in the medical field.
