Service Design Of Chronic Disease Management System Based On Community Medical Care

Chronic diseases have been a major problem afflicting the elderly group,in the context of China’s increasing aging population and deepening aging,how to improve the elderly group’s quality of life and happiness index is an urgent problem to be solved at present.Simple family or institutional pension is difficult to meet their special medical and pension needs,integrating pension service with medical service in the community has gradually become a trend.Taking the integrating pension service with medical service as the starting point and combining with the theory of service design,this paper studies the chronic disease management.The paper summarizes the related points of service design and the chronic disease management,and proposes to construct a multi-service ecosystem with all-round management and multi-party linkage.Taking cardiovascular disease as an example,the article studys and summarizes the demands of patients,medical staff and family members in the cardiovascular disease management services.Using the Kano model,article classifies the patients,demand types and calculates the comprehensive scores of the importance of each demand item,and further clarifies the correspondence between the demand items and the design key points in the service system which used to determine the design positioning and key points.Finally,the cardiovascular disease management service system is constructed and the mobile end design is carried out.
