Study On The Construction Of Touch Coding And Standardization Process In Non-verbal Communication Between Nurses And Patients

Objective.To explore touch as a kind of nonverbal communication behavior in medical encounter,its classification and the use of touch in nurse-patient communication through investigating elderly patients with the levels of its subtle power and emotional state and encoding the touch of nursing staff and patients experience.In order to analyze the correlation of touch and nurse-patient communication and develop standardized process of touching skills by the combination of quantitative research and qualitative research,which could help nurses to find the best timing for scientific skills intervention and improve the ability of negative power into positive,even promote disease rehabilitation.At the same time,it is effectively to reduce the incidence of disputes and establish the cooperative relationship between nurses and patients during the process of communication by touching.Methods.Through literature review,this study adopts questionnaire survey to the levels of subtle power and emotional state of 120 elderly patients of The third military medical university in the third affiliated hospital of before the operation;uses the method of observation,in-depth interview and focus group interviews with the combination of qualitative research methods,in-depth interviews with 18 nurses and focus group interview with 21 patients family members,using the theme and content analysis methods to analyze the theme of touch.According to the theory of interpersonal communication and Orlando's theory as the theoretical basis,touch with communication skills standardized intervention for elderly patients with power and emotion regulation process was developed during nursing process.Results.1.This research in ophthalmology,orthopedics and brain of elderly patients before the operation,positive negative emotion and anxiety level for the current situation of the baselinedata,which showed that at noon the day before the surgery,the power emotional state of elderly patients with eye visual deprivation is sad,the main characteristics of the orthopaedic falling accident elderly patients is miserable,the characteristics of the emotional state for elderly patients with brain hemorrhage is depressed.2.According to the status quo of the emotional state of the elderly patients and the use of touch during communication between nurses and patients,this research extracts 9 themes about the experience of using touch,as shown below:3.Based on 9 themes about the use of touch in the communication between nurses and patients,theory model of touch was improved.Abscissa for elderly patients with preoperative emotion type,from left to right is negative emotion type(sad,miserable,depressed),neutral mood type(sleepy,aroused,neutral),the types of positive emotions(happy,relaxed,excited);Ordinate for touch outcome,from the bottom to make patients with a negative emotional state into neutral even positive emotional state before operation.4."RPNAS" communication mode as the core steps to guide the communication between nurses and patients was built in this study:R(Relationship)— to build a harmonious communication atmosphere;P(Process)— to introduce the whole course of the disease to the patients and their famlies;N(Needs)— to ask the needs of patients;A(Adjustment)— to make both needs become same;S(Security)—to build a sense of Security on both sides.5.Through Delphi consultation method to improve the standard communication process.Results shows that using touch standardized process can effectively control the elderly patients with emotional state and make its negative emotion into positive emotional state,in order to improve patients compliance and establish better relationship between nurses and patients.Conclusion.Through the investigation of elderly patients with the levels of subtle power and emotional state,theory model of touch was analyzed.In the base of the theory of interpersonal communication and Orlando nursing process theory,"RPNAS" communication model was developed for different patients with different diseases and different emotional state.The study found that the neutral touch especially aroused touch is the "ice-breaking" relationship between nurses and patients.It is necessary to advocate nurses to applicate touch as the primary nonverbal communication skill on the clinical practice,which could help elderly patients regulate the levels of power and emotional state effectively.It is worthy to reduce the incidence of disputes and establish a harmonious relationship between nurses and patients provides a reference basis.
