Objective.To explore silence as a non-verbal communication behavior,its classification,role and the use of silence in nurse-patient communication,in order to identify and analyze different kinds of silence and build the use of silence model during medical encounter,and then provide a reference for healthcare workers to effectively respond to silence behavior.Methods.This study use qualitative research,combined with literature review,in-depth interview and focus group interview.150 intern-students' silence behavior was conducted through videotaped interviews during Objective Structured Clinical Examination,OSCE,combined with in-depth interview with 7 nurses and focus group interview with 5 patients and related family members.The coding systems used in these analyses were discourse analysis and content analysis.Based on the theory of Ida Jean Orlando's nursing process theory,through Delphi consultation method to build the silence Mnemonic of silence behavior coping-model during nurse-patient communication.Results.1.According to the use of silence in nurse-patient communication,this study extracts 7 themes about silence behavior during nurses and patients communication,as shown below:2.According to the 7 themes of silence behavior,this study divided silence into 3 categories,which are positive silence,neutral silence and passive silence.Among these themes,safety silence,issue resolved silence and empathy belong to positive silence;lack of interest silence,creating stress silence and embarrassed silence belong to passive silence;neutral silence can also divided into 2 small themes,which are privacy concerned silence and trust in medical technology silence.3.Built the silence Mnemonic of silence behavior coping-model during nurse-patient communication,through proposed a solution for silence behavior to provide a reference for nurses to effectively respond to silence behavior.4.Through Delphi consultation method to revise and make improvements to the silence-coping model.The results shows that take the 6 steps of effective communication as reference,using the model during medical encounter can effectively respond to different kinds of silence behavior,guide neutral silence to transform,encourage positive silence and make intervention to negative silence in order to build a better relationship between nurses and patients.Conclusion.This study based on the theory of Ida Jean Orlando's nursing process theory,using qualitative research to make a classification to silence behaviors during nurses and patients communication,so as to build the silence-coping model during medical encounter,which can provide a reference for nurses to effectively respond to silence behavior and reduce nurses and patients disputes.
