[Objective] With the development of the medical model, change of the concept of health, professional penetration of humanistic spirit and changes and extensions of nurse role, the importance of nurse-patient communication has become increasingly prominent. Pediatrics with a larger difference between pediatric and adult patients about physiological anatomy and immune system, the complexity of children’s disease and limitation of children’s language expression ability, is a special high risk of the department and prone to conflict of care (dispute). At the same time, due to lack of social and clinical experience, nursing students are prone to encounter nurse-patient conflict scene and often helpless. Therefore, it is necessary to carry out effective intervention to reinforce the nurse-patient communication ability of pediatric nursing students, which would provide them a kind of direct and effective communication method to deal with the possible conflict scenes quickly and correctly, and guard against the occurrence of dispute.[Methods] In this study, we selected 100 nursing students during May 2014 to March 2015 as the research object in Yancheng First People’s Hospital and Third People’s Hospital. They were 2012 level graduates of Yancheng Health Vocational and Technical College. According to drawing lots, the former 50 students were divided into the experimental group while later 50 into the control group. Students in the control group did routine clinical practice in accordance with the nursing internships outline. While the experimental group, in addition to conventional practice, was issued the questionnaire of "nursing intern communication language guide" and scheduled one hour to centralized learning and simulation training weekly on Monday, Wednesday and Friday afternoon. Moreover, we also arranged the experimental students on their own time to study the "Nursing intern communication term guidance" when they were being in clinical nurse-patient communication and the time was about one month. Finally, we using nurse-patient conflict scene nursing intern questionnaire and nursing student communication skills evaluation form, evaluated the nurse communication ability of the two intern groups by the method of self-assessment and her comment.[Results] 1. Based on clinical investigation and the depth interviews with nurses, we collected 56 nurse-patient conflict scenes prone to occur in the department of pediatrics. Moreover, after analysis and induction, we selected 32 scenes as the representative conflict scenes, each of which was defined according to the common problem principle that the same scene must have been brought 3 or more times.2. On the basis of 32 representative pediatric nurse patient conflict scenes, we compiled the questionnaire titled " the communication ability of nursing interns from Yancheng Medical College under the background of nurse-patient conflicting scenes", and employed the Cronbach’s alpha coefficient to evaluate the corresponding internal consistency reliability. As a result, the conflicting scenes could be divided into five classes according to the Medical expenses, the patient’s own or the condition of the disease, the doctors and nurses work not in place, the hospital environment facilities not in place and/or other factors and the value of the corresponding reliability coefficients were 0.912,0.945,0.938,0.917.0.924, respectively. The coefficients were above 0.7, which showed that the questionnaire had good internal consistency reliability. Besides, with 5 supervisors of nursing care effectiveness evaluation, the values of CVI were also above 0.84 and it also illustrated that the questionnaire had good validity.3. We compiled "nursing intern communication language guide", of which we also compiled the communication principle, the two communication paradigms and communication revelation reference for nursing students to every conflicting scene.4. After application of the nursing interns communication language guide, the nursing students were tested under the above 5 conflicting classes. As a result, according to the score, we found the experimental group (16.68±2.64,33.47±5.02, 24.45±4.16,21.55±3.56,37.63±5.18) was significantly better than the control group (13.74±3.18,28.56±6.74,20.03±4.35,16.94±4.11,29.82±6.82).[Conclusions] Our work through clinical survey firstly compiled the nursing students communication language instruction, of which every scene had the communication principle, contrast communication paradigms and communication enlightenment reference. Vivid communication paradigm would further make nursing students grasp faster and better. It can not only improve the ability of nurse-patient communication and help nursing students develop the confidence, but faster adapt to clinical condition and lay a good foundation for cultivating the ability to work independently. Meanwhile, the intervene study would widen field vision and be conducive for nursing students to solve the problem and improve their emergency ability in the face of sudden emergencies. In conclusion, our work shows that the communication language guide has strong clinical guidance when the nursing students are in the face of conflict scenarios.
