Objective1.To develop classic nursing-patient communication video cases based on clinical nursing work;2.To evaluate the effects of nursing-patient communication video cases on new nurses' communication skills training.MethodsThe study is divided into two phrases.The first part includes literature research and Delphi method.Based on the relevant literatures such as the nurses' ability to communicate with patients,the contents and methods of communication training,the characteri-stics and application status of the video cases,etc,the preliminary form of expert consultation questionnaire entitled "the classic nursing-patient communication scenario cases based on clinical nursing work" was initially formed.In order to ensure the authority and reliability of the nurse-patient communication cases,two rounds of expert consultation were conducted to evaluate the contents and methods of the cases from April to June 2016.1.Set up a research teamThe team consisted of three members,including one professor and two nursing postgraduate students.The research team was responsible for identify-ing the subject of consultation,designing expert questionnaire,selecting consultants,as well as collecting and analyzing the data.2.Design expert consultation questionnaireThe expert consultation questionnaire was designed on the basis of the previous literature research,it consisted of five parts:(1)Introduction:Briefly describe the purpose and significance of this study,the definition of Delphi method and acknowledgements;(2)Experts' basic information:Inclu-ding the age of the expert,the degree of education,professional title,research field and working experience;(3)Advice sheet of the nursing-patient communication cases:Including the notes and contents of the consultation;(4)The expert's familiarity with the questionnaire and the basis for judging;(5)Appendix.Specific contents of the nursing-patient communication scenario cases.3.Selection of consultantsInclusion criterias:(1)Bachelor degree or above;(2)Engaged in clinical nursing work for 10 years or more;(3)Associate professor title or above;(4)Interested in this study and volunteered to participate in it.4.Survey methodThe questionnaires were sent by the researcher and recollected as soon as possible within two weeks.After the first round of questionnaires were collected,the research team arranged the experts' opinions and conducted a statistical analysis.On the basis of the first round of expert consultation,the questionnaire contents were revised and improved to form the second round of consultation questionnaire.5.Statistical analysisSPSS17.0 statistical software was used for data analysis.Experts' basic information were represented by frequency and percentage Experts' positive coefficients were indicated by the recovery rate of consultation questionnaire and the response rate of each case;The concentration degree of experts'opinion expressed by the satisfaction mean:Kendall' w and coefficient of variation represented the coordination of experts' opinion.The second part is the empirical research.Using convenience sampling method,20 new nurses in a three-level hospital of traditional Chinese medicine in Changsha were selected as a research object and used the nursing-patient communication video cases for their communication skills training.A total of 21 hours(40min/class)were required for the training,2 times a week,each time for 3 classes and intervention about four weeks in all.The new nurses were evaluated with clinical communication skills assessment scale before and after the training,and the self-designed"nursing-patient communication video cases training feedback questionnaire"was used to evaluate the feasibility of nursing-patient communication video cases in improving their communication skills.Results1.Experts' basic information:The 17 experts in this study came from five three-level A-rate hospitals in Guangzhou and they were all female.The average age of the experts was(44.12 ± 5.51)years and the working years were(24.24±5.89)years;Experts with Bachelor degree accounted for 82.35%,Master degree accounted for17.65%;82.35%and 17.65%of them possessed a professional title of associate professor and professor respectively.2.Experts' positive coefficient:Two rounds of expert consultation issued 20 and 17 questionnaires,the effective recovery rate was 85%and 94.12%respectively.3.Experts' authority:It's determined by experts' familiarity and the basis for judgement about the questionnaire.The average authority coefficient of this study was 0.91,which indicated that the experts had high authority and the result was reliable.4.Experts' opinion coordination:The coordination coefficient of two rounds of expert consultation was statistically significant(P<0.001),and the values of Kendall' W were 0.164 and 0.315 respectively,which indicated that the experts' opinions were consistent and coordinated.5.Identification of the nursing-patient communication scenario cases:Through two rounds of expert consultation,the expert satisfaction rate of each case was 93.75%to 100%,and the satisfaction degree was 4.25-4.94.Eventually formed 36 scenario cases,including six aspects of nursing-patient communication:outpatient and emergency room,general ward,ICU and operation room,assist patients when checking,nursing treatment and special patients.6.The effects of nursing-patient communication video cases on new nurses' training.(1)Difference in scores of the new nurses' communication skills and scores of different dimensions before and after intervention.The results showed that the total score of nurses' communication skills after intervention was significantly higher than that before intervention(P<0.01).The scores of establishment of harmonious relationship dimension,listening attentively dimension,confirming the patients' problems dimension,mutual participation dimension and verifying feelings dimension before and after intervention were statistically significant(P<0.05).Only the scores of validating effective information dimension before and after intervention had no difference(P>0.05).(2)Evaluation of the effect of new nurses on nursing-patient communication video cases training.The results showed that 80%nurses were satisfied with the training effect.In the feedback survey project,scores ranked in the top five were:the training contents were practical,it provided the opportunity of self-presentation,it improved the awareness of the importance of nurse-patient communication,it improved the ability to communicate with patients,it improved the ability to analyze and solve the clinical problems,indicating that the vast majority of new nurses were satisfied with the training and confirmed the training effect.Conclusions1.This study has developed 36 classic nursing-patient communication video cases based on clinical nursing work by literature research method and Delphi method.Those video cases have good practicability and scientificity,and provide vivid case resources for clinical nurses and nursing students in communication skills training;2.Nursing-patient communication video cases have the advantages of vivid,intuitive,impromptu reflection,impressive,repeated observation and so on,which can effectively improve the new nurses' communication ability.These video cases can be used as effective training materials for clinical nurses and nursing students;3.The new nurses presented high satisfaction with the training of nursing-patient communication video cases and affirmed the training effect in general.
