Objective To assess the demands of empty-nesters during the transition from hospital to home to offer effective transitional care service for such patients.Methods The phenomenological research method in qualitative research are used in this study.12 empty-nesters discharged from two qualified AAA class hospitals and two community hospitals in Wuhan are selected by purposive sampling.By using a semi-structured in-depth interview,we find out and record the experience and demands of the empty-nesters during the transition.The data were analyzed by Giorgi analytical method.Results In this study,the demands of discharged empty-nesters for the extended nursing included 4 aspects:1 The professional health instruction:such as drug instruction,daily maintenance instruction,symptom identification instruction and so on.2 Informal service support:life nursing,emotional warmth,psychological guidance.3 Convenient service channel:health lecture,telephone visit,home visit.4 Available health resource:logical resource allocation,enhanced connection between hospitals,resource utilization of community hosptitals.Conclusion The research adapts to the situation of the aging and old people living separately trends in China.It takes the matter from empty-nesters and analyses their transitional needs in their home.Based on phenomenological research method,the study expresses the transitional needs finally by summing up the characteristic information from the raw data and summarizing the important points.The research results provide the theoretical basis for improving the quality of clinical teaching.will provide a scientific basis for medical staff to offer transitional care services and the government to make relative policies:Hospitals should expand contents and implementing form of transitional care and provide individualized service for them.And the government should collaborate large comprehensive hospitals with community medical centers to improve the quality of transitional care service.
