Objective:On the basis of the standard blood pressure treatment plan,the "protection motivation theory" was used to study the causes of poor blood pressure control in elderly patients with hypertension,and the effect of nursing measures based on nursing motivation on early morning blood pressure in hypertensive patients was analyzed to further improve the patients' early morning blood pressure.Method:The study was conducted in two phases.The first stage:the use of random sampling method,selected Jiangsu Provincial Hospital of Cardiology outpatient and inpatient early morning hypertension grade 1-3 patients 77,the use of self-made hypertensive patients with early morning blood pressure protection motivation questionnaire on elderly hypertensive patients related survey,Analysis,the establishment of hypertension in early morning blood pressure protection motivation theory framework.In the second stage,64 hypertensive patients who met the intervention criteria were randomly selected and randomly divided into the intervention group and the control group.The control group was given routine nursing.The intervention group was given routine Nursing Intervention for 3 Months Based on Protection Motivation Theory.The two groups were interviewed for 1 month after intervention and 3 months after intervention.Results:The first stage of the 76 patients through the survey to obtain the following results:1,Blood pressure:a single morning blood pressure systolic blood pressure or diastolic blood pressure?140/90 as the standard,there were 45 patients(59.21%)with non-conformity morning blood pressure.2,physiological indicators:26 cases(34.21%)patientswhose body weight index were<24,and 50 cases(65.79%)patients whose body mass index was no less than24 in in 76 elderly patients.Non-conformity rate of morning blood pressure in patients with a body mass index of no less than 24 were significantly more than those in patients with a body mass index of less than 24with statistically significant differences(P<0.05).3,health behavior:36 cases(47.37%)patients said that their home with a sphygmomanometer,40 patients(52.63%)patients do not have their own blood pressure monitor.Twenty patients(26.32%)had fewer patients who had measured blood pressure less than once a month.45 patients(59.21%)of the patients 3 months of active outpatient referral rate is less than once.42 patients(58.23%)had good compliance in 72 elderly patients with high blood pressure,and 30 patients(41.66%)had poor compliance and poor compliance.The rate of blood pressure in the morning was significantly higher than that in patients with poor compliance with statistically significant differences(P<0.01).4,The protection of motivation questionnaire score:The susceptibility score was 16.35±4.13.The severity score was 9.94±3.07.The internal reward score was 7.50±2.56.The external reward score was 5.50±2.06 points.The response performance survey score was 21.84±2.81 points.The reaction cost score of 16.09±4.37.The self-efficacy score of 15.72±3.08 points.second stage:1,General information:the control group and the intervention group of two groups of subjects sex,age,occupation,education,living,medical conditions,economic status,type of medication and other general demographic characteristics of the analysis,with on statistically significant differences between the two groups were no significant difference(P>0.05).2,early morning blood pressure:early morning systolic and diastolic blood pressure in patients in the trial group after 3 months intervention was lower than those of patients in the test group at baseline time with statistically significant differences(P<0.05 or P<0.01);early morning systolic blood pressure Diastolic blood pressure in patients in the trial group after 3 months intervention was lower than those of patients in the control group at baseline time with statistically significant differences(P<0.05).3,the health behavior:medication compliance,1 month blood pressure measurements,3 months the number of active referral,self-test blood pressure,since the blood pressure of the patient in the test group after 3 months intervention were better than those of patients in the test group at baseline(P<0.05 or P<0.01);medication compliance,1 month blood pressure measurements,3 months of active referral number,self-test blood pressure,since the blood pressure count of patients in the test group after 3 months intervention were Better than those of patients in the trial group at baseline time with statistically significant differences(P<0.05 or P<0.01).4,protection motivation questionnaire:susceptibility,severity,internal reward,external reward,response performance,self-efficacy score of patients in the trial group after 3 months intervention were better than those of patients in the test group at baseline time with statistically significant differences(P<0.05 or P<0.01);susceptibility,internal reward,external reward,response efficacy,self-efficacy score of patients in the trial group after 3 months intervention were better than those of patients in the control group at baseline time with statistically significant differences(P<0.05 or P<0.01);susceptibility,severity of patients in the control group after 3 months of intervention were better than those of patients in the control group at baseline time with statistically significant differences(P<0.05).Conclusion:1,the protection motivation theory can be reasonable in the elderly hypertensive patients in the early morning blood pressure control of the causes and factors,and can guide the development of the corresponding nursing measures.2,Nursing intervention measures based on the theory of conservation motivation can promote the improvement of early childhood blood pressure protection motivation,reduce the blood pressure in early morning and guide patients to improve drug compliance,improve health behavior and establish confidence in early morning blood pressure,reduce morning blood pressure.
