Objective To applicate Protection motivation theory in community elderly with hypertension to establish health behavior and control blood pressure.Methods This study used a quasi-experimental research methods and was performed in three stages.The first stage is to develope protection motivation questionnaire for elderly patients with hypertension.The reliability and validity of questionnaire were evaluated via Delphi method.In the second phase,300elderly patients with hypertension in Xuetian communities of NangTong were investigated with protection motivation questionnaire for elderly patients with hypertension.According to the survey,protection motivation model with elderly hypertension was established. In the third stage,the patients of second phase who met the inclusion criterion of third phase were random selected.72patients were selected in intervention group and72patients in control group. The subjects in the intervention groups received a6-momths comprehensive nursing intervention while the control group patients received only routine care. Results The results in the first phase was the protection motivation questionnaire for elderly patients with hypertension show good reliability and validity.Cronbach’s a was0.844,CVI was0.91-0.99,Pearson was0.87.The second phase:①The seven dimensions of the protection motivation questionnaire showed: Vulnerability was (15.65±4.30) points,severity was(10.56±2.33)points,internal and external rewards was(12.57±3.99) points,Response-efficacy was (22.35±2.66) points,Response-costs was(16.66±4.42) points,self-efficacy was(16.44±2.40)points;②Physiological indices of elderly hypertensive patients:triglycerides was(1.65±1.10)mmol/l,cholestero1was(5.54±1.31)mmol/l,SBP was(140.21±13.29)mmHg,DBP was(88.48±14.00)mmHg,body mass index was(25.68±4.37),waist circumference was(88.78±12.47)cm, hip circumference was(98.22±15.73)cm,abdominal circumference was(88.65±17.63)cm,31.29%of elderly patients with hypertension was high levels of triglycerides,60.79%was high levels of cholesterol,58.63%controlled blood pressure below150/90mmHg;③Others:46.04%had home blood pressure instrument.2.45%could measure blood pressure,7.19%monitored bood pressure daily,24.83%measured blood pressure once a week.,56.11%show good medication adherence, female patients with medication compliance were better than men (P<0.05).In the third Phase: Through6months of comprehensive nursing intervention based on protection motivation theory,promoted the elderly hypertensive patients to form protective motivation and healthy behavior.①Indicators of the protection motivation theory model:Severity, Vulnerability,Internal-rewards,External-rewards,Response-efficacy, Self-efficacy were statistically significant (P<0.05),while the Reaction of the price was not statistically significant (P>0.05);②Physiological indicators:cholesterol,SBP,DBP, waist-circumference were statistically significant (P<0.05),triglycerides,body mass index,waist circumference,hip circumference were no statistically significant(P>0.05);③Health behavior indicators:medication compliance,blood pressure measurement frequency,regular referral rate were statistically significant (P<0.05).Conclusions Comprehensive nursing intervention based on protection motivation theory can effectively promote community elderly hypertensive patients to form a protective motivation, a healthy behavior and control high blood pressure.
