Objective:The purpose of this study was to explore the effect of nursing intervention based on protective motivation theory on self-care ability,self-management behavior and blood sugar control level of elderly diabetic patients.In order to improve the application of this theory in clinical practice,it provides corresponding theoretical support.Method:In this study,126 elderly patients with diabetes who were hospitalized in the Department of endocrinology of a third-level hospital in Shanxi Province from May 2018 to October 2018 were selected as the research objects.The patients who were hospitalized in odd months were selected as the control group(63 cases),and the patients who were hospitalized in even months as the intervention group(63 cases).The patients in the control group and the intervention group were given routine nursing care.On this basis,the intervention group was given a three-month nursing intervention based on the theory of protective motivation.The intervention program was formulated for the patients from seven variables: severity,susceptibility,internal incentive,external incentive,self-efficacy,response efficiency and response cost.Through collective education and individualized guidance,the intervention group could help the patients.To help patients understand disease-related knowledge and the importance of rational diet,regular exercise,timely medication and regular monitoring of blood sugar in the prevention and treatment of diabetes mellitus,so as to promote patients to realize the seriousness of adverse behavior son the development of the disease and the susceptibility of their own adverse behavior to possible adverse outcomes;to make patients aware in the family and patient associationsorganized by researchers To the problems in the daily nursing process,recall and analyze the possible causes of this outcome with the patients,and solve the patients' problems in time.In the education of patients,we should make them realize their role in the treatment and nursing of patients,encourage and supervise the patients,and help them better control the development of diseases.We should hold an experience exchange meeting with patient,invite patients with better disease control in daily life to share their nursing experience,and help patients acquire useful knowledge from others' experience and experience.To help pat ients establish healthy behavior;to make patients understand that healthy behavior helps to reduce the economic and social costs of nursing.After discharging from hospital,patients adopt telephone follow-up to maintain their healthy behavior for a long time.Before and 3months after intervention,diabetes self-management behavior scale,self-care ability scale,fasting blood glucose and 2 hours postprandial blood glucose were used to evaluate the level of self-care ability,self-management behavior and blood glucose of the two groups before and after intervention.SPSS20.0 was used to establish the database for statistical analysis.The statistical methods included descriptive analysis and t-test.P < 0.05 indicated that there was significant difference.Result:(1)There was no significant difference in the basic data between the two groups(P >0.05).(2)The self-care ability,self-management behavior and blood sugar control of the two groups before and after the intervention were compared.After the intervention,the total score of self-care ability,self-concept,self-care responsibility,self-care skills and health knowledge of the intervention group were higher than those before the intervention(P <0.001).There was no significant difference in the scores of self-care ability,self-concept,self-care responsibility,self-care skills and health knowledge,diet nursing,exercise nursing,blood sugar monitoring,medication nursing and foot nursing before and after intervention in the control group(P > 0.05).After intervention,the scores of diet nursing,exercise nursing,blood sugar monitoring,medication nursing and foot nursing in theintervention group were higher than those before intervention,and the difference was statistically significant(P < 0.001).There was no significant difference in the scores of diet nursing,exercise nursing,blood sugar monitoring,medication nursing and foot nursing before and after intervention in the control group(P > 0.05).After the intervention,the fasting blood glucose and 2h postprandial blood glucose in the intervention group were lower than those before the intervention,and the difference was statistically significant(P< 0.001).There was no significant difference in fasting blood glucose and 2h postprandial blood glucose before and after intervention in the control group(P > 0.05).(3)There was no significant difference in self-care ability,self-management behavior and blood sugar between the two groups before and after intervention(P > 0.05);After intervention,the scores of self-care ability,self-management behavior and blood sugar control in the intervention group were significantly higher than those in the control group(P < 0.001).After intervention,the scores of diet nursing,exercise nursing,blood sugar monitoring,medication nursing and foot nursing of patients were higher than those of control group,and the difference was statistically significant(P < 0.001).The fasting blood glucose and 2 h postprandial blood glucose in the intervention group were lower than those in the control group,and the difference was statistically significant(P < 0.001).Conclusion:In this study,nursing intervention guided by the theory of protective motivation was implemented in elderly diabetic patients.Compared with traditional preaching and indoctrinating health education,health education guided by the theory of protection motivation is to stimulate Patient's self-protection motivation,improve self-care ability of diabetic patients,establish and maintain healthy self-management behavior,and actively carry out blood sugar management and control.
