Objectives To explore the influencing factors of trial of labor after cesarean delivery(TOLAC)by means of a retrospective analysis for maternal-neonatal outcomes of TOLAC and provide clinical evidences to parturition of TOLAC.Methodes A total of 387 patients with term pregnancy after cesarean section were divided into group trial of labor(group A,n=149)and group another cesarean delivery(group B,n=238).Group A were further divided into group vaginal delivery(group A1,n=119)and group failed trial of labor(group A2,n=30).The incidences of postpartum hemorrhage,puerperal infection and neonate asphyxia were compared.The influencing factors of successful vaginal birth after previous cesarean(VBAC)were screened.Results The incidences of postpartum hemorrhage,puerperal infection and neonate asphyxia had not any significant differences between Group A1 and Group B(p>0.05).The incidences of postpartum hemorrhage,puerperal infection and neonate asphyxia in Group A2 were higher than those in Group A1(p<0.05).The older gravida was,the higher postpartum hemorrhage risk was.There were significant differences in neonatal weight and,pre-pregnancy BMI,history of vaginal delivery,neonatal weight and cervical score between Group A1 and Group A2(p<0.05).However,there were no significant differences in interpregnancy interval(IPI),gestational age to labor and thickness of scar in lower uterine segment(p>0.05).History of vaginal delivery,neonatal weight and gravida age were vital influencing factors in successful VBAC.Conclusions VBAC was a safe and feasible delivery mode.Gravida age,pre-pregnancy BMI,history of vaginal delivery,neonatal weight and cervical score were vital influencing factors in successful VBAC.
