Objective: To explore the clinical efficacy and safety of IPL combined with the Alpha Hydroxy Acid in the treatment of moderate and severe acne.Methods: According to the diagnostic criteria and severity of acne,we selected 61 cases of moderate and severe acne from outpatients,all patients had completed the treatment,aged between 15~35.They were randomly divided into three groups: group A: 16 cases,group B: 23 cases,and group C: 22 cases.Before treatment we learned the basic information of all patients and explained account of matters to the patients carefully.All patients cleaned with their facial skin thoroughly before every treatment.The observer determined the lesion extent and type of the lesion,recorded the improvement of skin lesions,and evaluated the efficacy and safety of the patients.Group A: IPL combined with Alpha Hydroxy Acid treatment;Group B: pure Alpha Hydroxy Acid treatment;Group C: pure IPL treatment.We evaluated the finally curative efficacy of the fourth and eighth week after the end of treatment respectively.Results: These was no significant difference in gender,age,course of disease and severity of acne in three groups(P>0.05),it had comparable.On the fourth week after the end of treatment,16 cases of A group: 5 cases cured,7 cases markedly improved,3 cases improved and 1 case ineffective,the recovery rate was 31.25%,total effective rate was 75.00%;The reducing rate of inflammatory lesions and noninflammatory lesions was 74.00% and 65.00% respectively.23 cases in B group,3 cases cured,8 cases markedly improved,8 cases improved and 4 case ineffective,the recovery rate was 13.04%,total effective rate was 47.83%;The reducing rate of inflammatory lesions and noninflammatory lesions was 65.00% and 65.00% respectively.22 cases in C group,4 cases cured,10 cases markedly improved,6 cases improved and 2 case ineffective,and the recovery rate was 18.18%,total effective rate was 63.64%;The reducing rate of inflammatory lesions and noninflammatory lesions was 49.00% and 49.00% respectively.On the eighth week after the end of treatment,16 cases in A group,6 cases cured,8 cases markedly improved,1 cases improved and 1 case ineffective,and the recovery rate was 37.50%,total effective rate was 87.50%;The reducing rate of inflammatory lesions and noninflammatory lesions was 79.00% and 73.00% respectively.23 cases in B group,4 cases cured,8 cases markedly improved,8 cases improved and 3 case ineffective,and the recovery rate was 17.39%,total effective rate was 52.17%;The reducing rate of inflammatory lesions and noninflammatory lesions was 69.00% and 65.00% respectively.22 cases in C group,3 cases cured,8 cases markedly improved,8 cases improved and 3 case ineffective,and the recovery rate was 13.64%,total effective rate was 50.00%;The reducing rate of inflammatory lesions and noninflammatory lesions was 60.00% and 45.00% respectively.On the fourth and eighth week after the end of treatment,Compared the three groups,the curative effect of group A was superior to group B and C,in group A the effect of the inflammatory papules and pustules was the best,and inflammatory nodules diminished appreciably.The immediate reaction for the first time treatment with Alpha Hydroxy Acid(concentration 20%),14 patients' cheek or forehead in 39 appeared mild erythema,25 patients had moderate erythema(especially remarkable in the lesions)and relieved at the termination of treatment,all patients in the treatment had mild tingling,individual felt pain obviously,after multiple treatments there could be tolerated.16 patients with Alpha Hydroxy Acid treatment appeared dry skin,mild desquamation,and turned better after moistening,the amount of lesions in 4 cases increased after the first treatment,the lesions decreased significantly after multiple treatments.The IPL treatment response was consistent,all had transient erythema reaction,it lasted for several hours or faded after cold compresses,with no hyperpigmentation and hypopigmentation.Conclusions: The curative effect of IPL combine with Alpha Hydroxy Acid is better than separate treatment.The combination treatment of moderate and severe acne have a better synergy effect,short treatment time and almost no delay patients' work,small pain appeared in the process of treatment,patients had high adherence,and long-term curative effect.So IPL combined with the Alpha Hydroxy Acid in the treatment of moderate and severe acne is a kind of treatment for fast action and good curative effect,the adverse reaction can be controlled,it is worth promoting.
